Description:

The **Licensed Clinical Social Worker, Transplant** works as part of a multi-disciplinary transplant team, to provide individual counseling or referral for individual counseling as indicated by patient's assessment.

What you will do:
Works collaboratively with the behavioral health providers to include psychiatrists, psychologists and neuro psychologists for coordinated psychiatric patient care.
Provides biopsychosocial assessments, individual/family/group psychotherapy as needed as well as case management services as required.
Support the referrals from providers in the ambulatory setting seeking mental health, social and financial services for their patients.
What you will need:
+ Preferred two years acute hospital or transplant experience MSW from a CSWE accredited School of Social Work State of Illinois
+ LCSW; Licensed Clinical Social Worker State of Illinois
Compensation:
Salary Range: $29.07 - 46.51per hour
