Description:
Job Title:** **Social Worker – Homecare/Hospice
**Reports To:** Professional Services Manager or Designee
**FLSA Status:** Exempt / Non-Exempt
**Pay Grade:** 12
Summary
Delivers medical social services to homecare and hospice clients under physician orders and supervision of a Master’s-level Social Worker. Supports clients and families in managing social, emotional, and environmental challenges related to illness and care.
Qualifications
+ Bachelor’s in Social Work (BSW) from an accredited program
+ 1+ year of healthcare social work experience
+ State registration/licensure (as required)
+ Valid driver’s license and reliable transportation
+ Ability to work under supervision and within interdisciplinary teams
Key Responsibilities
+ Assess client needs and develop care plans
+ Provide counseling and connect clients to community resources
+ Collaborate with care teams and participate in treatment planning
+ Document care activities accurately and timely
+ Maintain compliance with regulations and agency policies
+ Support quality improvement and professional development initiatives
Work Requirements
+ Frequent travel to client homes
+ Ability to lift, stand, and perform physical tasks as needed
+ Adapt to changing priorities and work environments
+ pay grade range- $31.73-$50.77
