    Are you an organized, detail-oriented communicator who's great at building trust with families and community partners? Do you have experience helping people navigate services for individuals with developmental disabilities-especially through our Office for People With Developmental Disabilities (OPWDD)? Lifetime Assistance is looking for a compassionate and reliable Intake Specialist to support individuals as they transition into our residential programs.

    What You'll Do:

    As our Intake Specialist, you'll lead the intake process from start to finish-reviewing referrals, guiding families, coordinating with care teams, and ensuring smooth, timely admissions into residential care.

    Key responsibilities include:

    + Managing referrals and admission coordination

    + Guiding families through the intake process with empathy and professionalism

    + Collaborating with hospitals, care managers, and service providers

    + Maintaining accurate documentation in our Electronic Health Record (EHR) system

    + Organizing intake meetings and communicating next steps clearly

    + Tracking referral activity and ensuring compliance with state and agency guidelines

    What We're Looking For:

    + Experience working with OPWDD or in intake/admissions is strongly preferred

    + Excellent communication skills-you'll lead sensitive conversations with families and agencies

    + Highly organized, with strong attention to detail and follow-through

    + Familiar with EHR systems and general documentation standards

    + Bachelor's degree in Human Services or related field preferred

    + Must meet Lifetime Assistance's vehicle operator requirements
