Position OverviewUnited Health Services (UHS) is seeking a Field Nurse to provide high-quality, compassionate care to patients in their homes. This role is essential to supporting patients with ongoing health needs, ensuring continuity of care outside a clinical setting. The Field Nurse will assess patient conditions, administer treatments, educate patients and families, and coordinate with other members of the healthcare team to deliver personalized, effective care.
This position requires strong clinical judgment, excellent communication skills, and the ability to work independently in a community-based setting. The ideal candidate will demonstrate flexibility, empathy, and a commitment to patient-centered care while representing United Health Services in the field.

$33.57 - $51.39 per hour, depending on experience

Job Responsibilities
+ Conduct initial assessment visits in patients’ homes to determine eligibility for home health services and develop preliminary care plans.
+ Obtain physician orders and approval for the proposed plan of care.
+ Provide skilled nursing care in the home setting in accordance with physician orders, nursing standards, and agency policies.
+ Educate patients, family members, and caregivers on care routines and therapeutic regimens to promote health and independence.
+ Implement and coordinate individualized plans of care in collaboration with other healthcare professionals.
+ Monitor and re-evaluate the patient’s condition, reporting changes to appropriate care team members.
+ Follow up on identified clinical concerns to ensure continuity and effectiveness of care.
+ Advise patients and families on available community resources, especially related to post-discharge care needs.
+ Participate in interdisciplinary case conferences and contribute to care planning.
+ Supervise and orient home health aides in line with agency standards.
+ Complete all required documentation accurately and in a timely manner.
+ Assist in onboarding and mentoring RN staff and nursing students.
+ Maintain and enhance professional knowledge through continuing education and training.
+ Participate in quality improvement initiatives within the agency.
+ Adhere to compliance regulations and attend required corporate compliance training.
+ Protect patient and employee confidentiality in compliance with HIPAA and agency policies.
+ Perform other duties as assigned.
Position Qualifications
Minimum Required:
+ Graduate of an approved RN program OR a non-approved program supplemented by content equal to an approved program.
+ Two years' experience as a Registered Professional Nurse required OR equivalent of education and experience.
+ License and current registration to practice as a Registered Professional Nurse in New York State.
+ CPR Certification required upon hire.
Preferred:
+ Bachelor's degree in nursing
+ Med/Surg/Certified home care experience.
