We are seeking a talented Restoration of Self Sufficiency Facilitator to join our team of nearly 500 employees. The Restoration to Self Sufficiency Facilitator meets with Temporary Assistance Recipients who are unable to work due to a disability. The purpose of the program is to assist clients in navigating community resources and addressing barriers to employment so that they can move towards self-sufficiency.
CCSI is partnering with RSSP to hire aRestoration of Self Sufficiency Facilitator.This position is employed by CCSI and working on behalf of RSSP.
In this role, you will
+ Conduct initial interviews with clients that Monroe County Department of Human Services (DHS) has identified as being unable to work due to a physical or psychological condition
+ Work with clients to develop a Restoration to Self Sufficiency Plan that outlines how they can best address barriers to employment
+ When appropriate, connect clients to community resources like physical or mental health providers, care management, child care or transportation resources
+ Monitor attendance at scheduled physical and mental health appointments and follow up with clients when they do not attend recommended appointments
+ Maintain written and electronic client records
+ Performs other duties as assigned
Requirements
What You Bring
+ Minimum two years of experience in behavioral health field and/or customer service
+ Associate Degree in a Human Service field preferred
+ Prior experience in vocational rehab or a general knowledge of community resources would be a plus

Required for this position are:
+ Satisfactory completion of all required background screenings.
+ Respect the confidential nature of all CCSI and CCSI customer information you are exposed to in the course of your work performance. Staff working directly with protected health information /electronically stored protected health information, personally identifiable information, and other patient/consumer/customer data, will abide by the specific procedures and policies outlined by CCSI’s Code of Conduct and CCSI’s Privacy and Security Policies, New York State regulations and Federal regulations.
