    Homelake Veterans Center

    The Colorado State Veterans Center at Homelake is a State-owned and operated skilled nursing facility and domiciliary dedicated to serving those who have served us. We strive to provide excellent customer service and to respect and honor resident choices regarding bathing, rising, bedtimes, and other personal decisions. Our philosophy is to provide residents a home where life and love abound.

    Description of Job

    The Client Care Aide II will t ransport Residents with floor coverage as needed when no transports are scheduled. Provide basic nursing care to residents of varying mental, physical and health status’s. The position performs or assists in meeting the needs, ie., activities of daily living, performs ambulation, range of motion, vital sign assessments, etc.

    Job Duties:

    + Transportation in a State vehicle to all appointments scheduled for SNF and DOM residents.

    + Performs basic nursing care; bathing, feeding, dressing, transferring, ambulating, positioning, lifting, range of motion, toileting, skin care, etc

    + Safety of residents and other by use of correct lifting techniques, monitoring the environment and Resident whereabouts/activities, use of safety devices such as lifts and enabler bars, etc.

    + Performs basic resident assessments such as weights, bowel and bladder incontinence, intake and outputs, vital signs, and records the same

    + Communicates with others to ensure proper resident care and work completion

    + Keeps RN’s and LPNs informed of findings and changes in resident condition

    + Transports residents for medical appointments, etc. in state vehicles when the need arises

    + Attends in-house and off-site educational training, cornerstone online training and staff meetings to maintain current on techniques, policies adequate inter-shift communication

    Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

    Special Qualification:

    + Current & active Certified Nurse Aide license renewal required and be CPR certified, Vaccinations as required by State and facility.

    Education/Licensure/Certification/Experience:

    + High school diploma or General Educational Diploma (GED)

    + One year of experience in an occupational field related to the work assigned to the position

    Substitutions:

    + Current, valid certification as a Nurse Aide (CNA) from the Colorado Department of Regulatory Agencies will substitute for the high school diploma or GED

    + Current, valid licensure as a Practical Nurse (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact will substitute for the experience requirement

    Conditions of Employment:

    + CDHS employees (all Direct contact with vulnerable persons):

     

    CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services),Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers)

     

    + Current/Valid and unrestricted US Driver’s License

    + Off-site training, transportation. Transports residents for medical appointments, etc. in state vehicles when needed

    + Shift Work: Weekends, Holidays, evenings, nights and days

    + Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
