What You’ll Do:
As a Hospice On-Call RN, you’ll play a vital role in delivering high-quality, patient-centered care. You’ll assess, plan, and implement nursing care in alignment with the interdisciplinary team, ensuring patients receive the support they need—when they need it most.
Key Responsibilities:
+ Conduct comprehensive physical, emotional, and spiritual assessments
+ Collaborate with physicians, families, and the care team to develop and implement care plans
+ Provide education and emotional support to patients and caregivers
+ Respond to urgent needs during your shift, including admissions, symptom management, and crisis support
+ Maintain accurate and timely documentation
+ Participate in quality improvement initiatives and team meetings

What We’re Looking For:
+ Current RN license in good standing-IL
+ Hospice or palliative care experience required
+ Strong critical thinking and communication skills
+ Ability to work independently and as part of an interdisciplinary team
+ Commitment to compassionate, patient-centered care
+ Pay range:$$35.80-$57.28
