    The Vice President of Population Health oversees enterprise-wide operational execution and transformation; aligns operations with corporate strategic priorities and client trends. The VP is responsible for creating organizational strategy for care management programs to reduce the total cost of care for the CKD/ESRD population. The VP plays a critical role in client relationships; drives integration with various clients and fosters relationships with counterparts that lead to program and P&L growth.

    Responsibilities

    Essential Duties and Responsibilities:

    + Oversees enterprise operational strategy, ensures operations, processes and systems are flexible and adaptable to meet client needs, organizational goals, and industry demands.

    + Champions enterprise-wide process modernization; continuous improvement strategies, drives scalability. Guides other departments with resource focus/allocation to enable operations success.

    + Shapes long-term organizational strategy; drives innovation in care model design, client differentiation of care models.

    + Leads program innovation roadmap; collaborates across departments to evaluate strategy and success of care model and influences decisions in model changes based on financial and clinical outcomes.

    + Oversee the development and practice implementation of population health initiatives and continue to optimize as needed to achieve the desired outcomes.

    + Accountable for total client portfolio financial performance; leads risk optimization and payer/provider contract strategies.

    + Provides input on contract financial modeling and proposals. Helps set contractual terms and targets, provides expertise on care model changes to meet client demands.

    + Develops executive-level relationships with clients, supports team in building client strategy; drives external engagement and reputation.

    + Influences client and organizational strategy through data-driven insights; owns roadmap for new care model changes.

    + Coaches and mentors Directors, Senior Directors and AVP’s in setting strategy to meet KPI targets and path to achievement and establishes best practices related to initiatives to determine short-term and long-term success measures.

    + Leads cross-functional teams in performance improvement and profitability strategy.

    + Conduct research and interviews to identify best practices and develop a deep understanding of how others are addressing related problems.

    + Stay at the forefront of value-based care to gain innovative ideas and approaches.

    + Perform all other duties and special projects as needed to support the overarching goals of the population health team.

    Qualifications

    Required Education and Experience:

    + Bachelor’s degree in health/business/engineering field and/or

    + A master’s degree or other advanced degree in nursing, social work, business, health services research, health policy, information technology, or other relevant fields

    + 10-15 years of related experience, preferably in population health operations, ambulatory care, community public health, case, or care management, or coordinating care across multiple settings and with multiple providers

    + Direct P&L Responsibility $50M **_Preferred_**

    + Relevant competencies include clinical program development and improvement, analytical problem solving, project management, effective written and verbal communication, and ability to positively influence others.

    + Knowledge and experience with care management competencies

    + Experience working with vulnerable populations (geriatrics, minorities, behavioral health)

    + Ability to navigate ambiguity with the aid of structured problem-solving techniques.

    + Must be results-oriented with the ability to prioritize and manage multiple projects simultaneously and coordinate activities to consistently meet deadlines and client expectations.

    + Comfortable working in a fast-paced, dynamic environment while maintaining focus on key goals.

    + Familiarity with health care laws, regulations, and standards.

    + Understanding of coding and reimbursement systems, risk management and performance improvement is helpful.
