Our client in Atwater, CA seeks a dedicated, passionate Physical Therapist to provide care to patients in the Atwater/Merced, CA area.

Experience as a PT is preferred but if you are in the beginning stages of your career with less than a year experience, you are still welcome to apply.

POSITION DETAILS:

+ Pay: $3,500 per week. Weekly Travel Stipends included. Must live 50+ miles away to qualify to stipends.

+ Location: Atwater/Merced, CA

+ Assignment Length: 13 or 26 weeks

+ Schedule: 4x10s. Flexible schedule (Mon-Thurs, Tue-Fri, Wed-Sat)

+ Transportation: Reliable transportation required. Mileage reimbursed at $0.67 per mile. Travel between homes is approximately 75-100 miles per week.

JOB DUTIES:

+ Obtains all pertinent medical history from patient, family, or significant others

+ Performs PT assessment visit and documents accordingly

+ Performs home safety check and environmental assessment of the patient’s home environment

+ Helps develop an appropriate and effective PT Plan of Care (POC) to be submitted to the physician for approval and implementation

+ Functionally reassess the patient for on-going therapy

+ Determines medical necessity for other services

+ Evaluates the patient’s ADL and iADL abilities

+ Effectively communicates ongoing with patient and family regarding the POC and progression of such

+ Effectively instructs patients and families in the safe use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, mechanical lifts etc)

+ Effectively communicates with other disciplines in the case

+ Communication with the patient’s physician (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities

+ Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests

+ Submits ALL OASIS assessments/documentation within 48 hours of the OASIS visit

+ Participates in staff meetings and in-services

+ Maintains professional licensure and actively seeks out educational experiences to enhance the practice of home care physical therapy and the benefit of the patients serviced

+ Other duties as assigned

REQUIREMENTS:

+ Current CA Physical Therapist License

+ Current BLS

+ 1 year Home Health experience.

+ Reliable transportation (Mileage reimbursed)
