    For Water Street Mission to fulfill our mission and purpose, it is critical for all staff to participate in the development and maintenance of a healthy Christian Community.     Job Summary: Serve as a member of the Water Street Health Services team to achieve the primary goal of the administration and oversight of recuperative care. Provide direct nurse care management services for recuperative care guests.

     

    FLSA: Full time (40 hours), Exempt

    All Water Street Mission staff must be:

    Essential Functions:

    Culture and Values

     

    + Share the Good News of Jesus Christ in words and actions with people we serve, communicating hope and dignity.

    + Walk alongside fellow WSM staff in Christian community as we journey toward maturity.

    + Encourage restoration in the lives of guests, recognizing we are each restored by God to become restorers.

    + Work in full coordination with other WSM staff to identify, report and respond appropriately to any potential threats to the safety and wellbeing of guests.

    + Embody, encourage, and cultivate WSM culture, purpose, mission and values in words and actions.

    + Communicate and demonstrate care for the well-being of staff and a flourishing WSM culture that values employee engagement and resilience within a thriving environment.

     

    Role Specific Responsibilities

     

    + · Extensive knowledge of, and adherence to, RC admission criteria including screening and referral.

    + · Orientation of guest to RC services and facility, including admission agreements and discharge planning.

    + · Management and oversight of all RC services, including quality improvement.

    + · Establish annual budget and management of resources.

    + · Maintain up-to-date policies and procedures for RC.

    + · Identify potential clinical and supportive service needs upon admission.

    + · Develop care plan based on guest care needs and goals.

    + · Coordinate care and facilitate connections with/to appropriate providers.

    + · Connect guests to primary care and community health providers, when appropriate.

    + · Assist guests in planning for, and navigating, post recuperative transportation needs.

    + · Assist with prescription medication management and safe storage.

    + · Knowledge and training to implement trauma informed care principles to all aspects of care.

    + · Establish service benchmarks and documentation of identified data.

    + · De-escalation training and crisis prevention strategies

    + · Person centered care that exemplifies dignity, humility, and respect.

    + · Knowledge of, and adherence to, health information privacy and HIPAA regulations.

    + · Establish goals focused on maximizing functioning within the community and decreasing the need for emergency care.

    + · For urgent issues, after business hours phone consultation as needed, duty to be shared with WSHS staff.

    + · Participate in planning and other administrative meetings as needed

    + · Assist in promotion of Recuperative Care through conducting tours, thanking donors and building community relationships

    + · Attendance and participation in Mission Director, MDT and WSHS Leadership meetings

    + Other duties as assigned.

    + · Supervision and management of recuperative care staff.

    + q Be able and willing to share the Gospel and participate in the ministry activities of Water Street Mission.

    + q Subscribe to the Water Street Mission Statement of Faith and Qualifications of Employment upon hire and continuously while employed.

    + q Adhere to the Water Street Mission Employee Handbook.

     

    Supervision

     

    Education and Experience: Bachelor's degree and a minimum of 5 years' experience in the nursing field. Management experience required. Unrestricted License to practice nursing in Pennsylvania. Experience working with patients that have a history of trauma. Experience working with Substance Use Disorder and Mental Health preferred. Experience growing relationships and partnerships across an organization and with community services.

     

    Reports To: Senior Vice President, Health Services

     

    Key Internal Relationships: Director of Medical Services, Director of Behavioral Health Services, Residential Life, Community Resources, Director of Social Work, Access Center; Supervisors throughout WSM

    Physical Demands:

    Occasionally = 1%-33%; Frequently = 34%-66%; Continuously = 67%-100%

     

    The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this position.
