    As a Medication Access & Care Coordinator, you'll be part of the Medication Care Program Team that ensures patients with complex medication needs receive timely, affordable, and well-coordinated pharmacy care. This role is essential to the success of our Medication Care Program, which supports patients navigating chronic conditions, multiple therapies, or challenging access barriers.

     

    You’ll coordinate care across patients, providers, pharmacies, and payors—using technology, communication skills, and clinical insight to keep therapies aligned and avoid unnecessary disruption. This includes identifying opportunities to simplify medication schedules, engaging patients in ongoing support, and ensuring financial and billing pathways are clear and accurate. While the work is rooted in a pharmacy setting, it involves much more than processing prescriptions—it's about removing barriers before they happen and giving patients peace of mind.

     

    This position is Monday through Friday, approximately 8:00 a.m. to 4:30 p.m., with no weekends or holidays. The role may be onsite at the Parkview Integrated Pharmacy or Medical Park 11 Pharmacy, with an option for being hybrid in the near future.

    License Requirements: Must have or be eligible for a State Board of Pharmacy Tech in Training license or a current State Board license as a Pharmacy Technician or Intern for the State where services will be provided.
