General Summary
The Behavioral Health Clinic Coordinator will primarily serve in the role of Autism Program Manager for CADD's Autism Diagnosis Clinic (ADC). The ADC is a grant funded project through the Pennsylvania Department of Health that will increase access to a diagnostic evaluation and parent coaching program for autism for patients 18 months - 3 years living in select counties in Pennsylvania.
The Autism Program Manager will have opportunities to provide both clinical and administrative support services to enhance patient care by performing a variety of duties. The Autism Program manager will be responsible for coordinating with select county Early Intervention (EI) providers, Intermediate units (IU), and medical providers for patient enrollment into the ADC. The Autism Program Manager will play a key role in enrolling patients into the ADC and coordinating their care throughout the ADC. This will include, ensuring that all information is collected from patients prior to the diagnostic evaluation, interviews with caregivers to obtain developmental histories, scheduling of diagnostic and parent group sessions, and collaborating with CADD's diagnostic team.

Essential Functions:
+ Interviews patients and family members to obtain information about home environment, family relationships, health history, and patient's personality traits.
+ Conducts individual or group therapy sessions. Assesses patients' progress and modifies or extends treatment program accordingly.
+ Provides therapeutic intervention with patient and family including individual, marital, parental, family and group therapies.
+ Documents current, accurate observations of care to the patient in appropriate records.
+ Completes warm hand-offs to/from providers and assists with transitions between outpatient services and helps with care coordination.
+ Provides brief therapy interventions, case management and community linkage and crisis management, both virtual and in person.
+ Completes follow up contacts between appointments.
+ Completes safety assessment and safety planning with patients and families.
+ Send applicable correspondence when necessary.
+ Works collaboratively with team leads across outpatient offices and clinical teams.
+ Keeps abreast of new developments by attending meetings and/or workshops as appropriate.

+ Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
Qualifications
Minimum Education:
+ Bachelors Degree From a college or university that is accredited by an agency recognized by the United States Department of Education or the CHEA in a recognized clinical discipline including social work, psychology, nursing, rehabilitation or activity therapies. Required or
+ Bachelors Degree Has an equivalent degree from a foreign college or university that has been evaluated by the AICE or the NACES. The Department will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency. Required or
+ Masters Degree Has a graduate degree in a clinical discipline with 12 graduate-level credits in mental health or counseling from a program that is accredited by an agency recognized by the United States Department of Education or the CHEA. Required
Work Experience:
+ 1 year Experience in a health or behavioral health-related office with specialized skills and knowledge. Required
Licenses:
+ Basic Life Support Upon Hire Required
Courses and Training:
+ Must provide clearance per the Child Protective Services Law amended by PA Act 33 of 1985. Upon Hire Required
Knowledge, Skills, and Abilities:
+ Excellent human relations and oral/written communication skills.
+ Analytical abilities.
+ Computer and keyboard skills.
+ Knowledge of and ability in the care and handling of patients in all age groups.
+ Knowledge of PDMP, Lexicomp, Lippincott, Systems of Care, eCare, community services and/or other physician support systems.
+ Proficiency in Therapeutic Crisis Intervention (TCI) principles of de-escalation.
+ Knowledge of EPIC.
+ Knowledge of Insurance.
