Job Description
We are looking for a **Wellness Program Coach** to support the Wellness Program for a specific group of members by actively engaging them via telephone. This position will be **Part-Time** and Remote, starting at 20 hours per week, increasing to 40 hours per week as demand increases.
Due to the specific legal and contractual requirements associated with this position, only US Citizens will be considered for this role. Candidates must be able to work directly on CAI's W2.
What You’ll Do
+ Engage members via telephone to assess their needs and biometric screening results
+ Provide scheduled coaching sessions, guiding members in creating personalized care plans, supporting lifestyle modification, decreasing health risk behaviors, connecting to resources, and navigating the healthcare system
+ Communicate effectively using active listening, motivational interviewing, and knowledge of behavior change strategies
+ Utilize knowledge of evidence-based interventions and industry standards to deliver high-quality lifestyle coaching in areas such as tobacco cessation, weight management, sleep, stress management, nutrition, and physical activity
+ Assess individual needs by evaluating current health status, facilitate discussion around biometric screening results, identify areas for improvement, and establish personalized goals
+ Co-create personalized care plans with members with actionable steps towards achieving their lifestyle goals
+ Provide support and guidance through offering encouragement, education, and connecting to internal and external resources
+ Facilitate behavior change by helping members make lifestyle changes and overcome obstacles to long-term success
+ Collaborate with peers, case management, and multi-disciplinary teams to holistically support member needs
+ Ensure all activities are documented and conducted in compliance with business process requirements
+ Work within a fast-paced team environment
+ Attend and participate in required meetings
+ Other duties as assigned or requested
What You'll Need
Required:
+ Bachelor’s degree in a health-related field (nutrition, health education, health science, health promotion, exercise science, public health)
+ A minimum of 2 years’ experience in a role that supports the management of health services and/or wellness programs
+ Telephonic coaching experience
+ Comprehensive knowledge and understanding of current health education practices
+ Knowledge of Prochaska’s Transtheoretical Stage of Change model
+ Excellent interpersonal, relationship, and consensus-building skills
+ Ability to work in a high-performing and flexible team environment
+ Demonstrated ability to handle multiple priorities in a fast-paced environment
+ Excellent organizational and time management skills
+ Self-directed; self-starter, ability to work successfully with indirect supervision and moderate autonomy
+ Proficient in basic computer skills and software applications including Microsoft Teams
+ Communication skills, strong relationship building, highly organized, adaptable, and flexible, ability to work in a fast-paced environment
Preferred:
+ Master’s degree in a health-related field (nutrition, health education, health science, health promotion, exercise science, public health).
+ Certified Tobacco Cessation Specialist (CTTS)
+ Registered Dietitian
+ Certified Diabetes Care and Education Specialist (CDCES).
+ Exercise Physiology certification
+ National Diabetes Prevention Program Lifestyle Coach
+ Certified Health and Wellness Coach (CHWC)
Physical Demands
+ Ability to safely and successfully perform the essential job functions consistent with the ADA and other federal, state, and local standards
+ Sedentary work that involves sitting or remaining stationary most of the time with occasional need to move around the office to attend meetings, etc.
+ Ability to conduct repetitive tasks on a computer, utilizing a mouse, keyboard, and monitor
The pay range for this position is $32.00- $34.00 per hour (USD). Exact compensation may vary based on several factors, including location, experience, and education. Benefit packages for this role includes medical, dental, and vision insurance, as well as 401k retirement account access. Employees in this role may also be entitled to paid sick leave as provided by applicable law.
