    About the Role:

    As a Care Coordinator, you will be responsible for managing day-to-day care staffing for clients in both our In-Home (Kansas) and HCY (Missouri) programs. You’ll work closely with families, caregivers, and clinical teams to ensure quality care, timely communication, and continuity of services.

    What You'll Do:

    + Coordinate caregiver schedules based on client needs and caregiver qualifications

    + Build meaningful relationships with families and caregivers to ensure trust and care satisfaction

    + Maintain accurate documentation and reports per program guidelines

    + Communicate clearly and professionally across departments and with external partners

    + Assist with onboarding and training new caregivers to ensure a high standard of care

    + Respond to staffing needs quickly and creatively to ensure client support

    + Represent Phoenix with professionalism and empathy at all times

    What You Bring:

    + A true passion for working with children with special needs and the elderly

    + Experience in home care, case coordination, scheduling, or human services preferred

    + High school diploma or equivalent required

    + Ability to multitask, problem-solve, and adapt quickly in a fast-paced environment

    + Strong communication and organizational skills
