Join the MercyOne Family! We are looking to hire a RN Case Manager on our Med/Surg/ ICU units

As a RN Case Manager at MercyOne, you work with the interdisciplinary team for patients on a select unit from admission to discharge. This care will be determined by individualized plans of care, care guidelines, best practice data and critical paths.

Schedule:

+ full time .9 FTE

General Requirements:

+ Mobilizes resources and manages the systems to achieve outcome-based care

+ Coordinate physician, patient and payer interactions to ensure appropriate level of care.

+ Respond to concurrent and retrospective data analysis identifying trends in patient care for that unit’s patient population.

Education:

+ Active IA RN license

+ BSN preferred

+ Minimum of 4 years of clinical experience.
