    **Master Social Worker (MSW)** – Home Care -Part Time

     

    Territory: Northern Chicago – Maywood, Oak Park, Forest Park, Melrose Park, Elmwood Park, Franklin Park, Schiller Park, Schaumburg, Carol Stream, and Chicago ZIP codes north of North Ave.

     

    Join Loyola Medicine Home Care and Hospice, a member of Trinity Health At Home, as a Master Social Worker (MSW). In this meaningful role, you’ll provide compassionate, one-on-one care to patients in their homes, helping them navigate emotional, psychological, and safety challenges during their care journey.

    What You’ll Do

    + Conduct initial assessments to identify patient needs

    + Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams

    + Connect patients and families with appropriate community resources

    + Ensure compliance with federal, state, and local regulations

    + Participate in quality improvement initiatives

    Qualifications

    + Master’s Degree in Social Work (MSW)

    + Licensed Social Worker in the State of Illinois

    + Minimum 1 year of experience in a healthcare setting (home care/hospice preferred)
