The Division of Infectious Diseases is seeking a **_Senior Clinical Nurse_** . The primary purpose of the Early Intervention Services (EIS) Senior Clinical Nurse is the provision of HIV treatment and prevention nurse case management services for patients who are seen in the John G. Bartlett Specialty Practice (JGBSP). The EIS Senior Clinical Nurse is crucial to the Bartlett HIV program and will report directly to the Clinical Nurse Manager.
The primary responsibilities of the EIS Senior Clinical Nurse include
+ Linkage to care and nurse case management of people newly diagnosed and previously diagnosed with HIV and people referred for PrEP (pre-exposure prophylaxis) services who are engaging in care at JGBSP.
+ Day-to-day management of the Rapid Start/Restart HIV treatment and prevention, and HIV testing and counseling programs at JGBSP, and updating protocols, conducting quality control and improvement activities, and assisting with grant writing and reporting for grants that support these programs.
Specific duties and responsibilities
+ Rapid Start/Restart HIV treatment and prevention, and HIV testing and counseling programs at JGBSP:
_Program Management Duties_
+ Lead with the oversight of the day-to-day operations of the Rapid Start/Restart and HIV testing and counseling programs.
+ Work closely with the Ryan White Director to complete grants and required narrative reports for funds that support the Rapid Start/Restart and HIV testing and counseling programs.
+ Work with the Ryan White data manager and Bartlett HIV program team to develop and implement data collection protocols, internal and external PrEP and HIV treatment referral protocols for high-risk HIV negative individuals and patients linking and re-engaging in HIV care, entry and retention protocols for Bartlett clinic PrEP and HIV-positive patients.
+ Establish Rapid Start/Restart and HIV testing and counseling program goals, objectives, and monitor progress.
+ budget for patient education materials, office supplies, and other program functions.
_Early Intervention Services (EIS): Nurse Case Management Duties_
_HIV rapid start activities_
+ Point of contact for patients newly diagnosed/previously diagnosed, reengaging care.
+ Screen for eligibility for rapid start/re-start of ART.
+ Rapid start visits care coordination.
+ HIV nurse intake/assessment with warm hand-off to rapid start HIV provider, case manager, and navigator.
+ HIV and ART education and adherence counseling.
+ Tracking of patients who receive rapid start to help ensure transition to HIV continuity care.
+ Point of contact for rapid start collaborative activities with the Johns Hopkins Emergency Department, inpatient unit, JHH, and external agencies where HIV testing occurs.
_PrEP and HIV testing, and counseling activities_
+ PrEP nurse intake and PrEP eligibility assessment.
+ HIV and PrEP education and counseling, case management screening.
+ Phone/video and walk-in triage.
+ Medication adherence counseling.
+ Walk-in HIV testing, counseling, and linkage to care services (PrEP and HIV treatment).
_Reengagement in care activities_
+ Review a monthly report from the electronic medical record (EPIC) run by the Bartlett data manager that identifies HIV-positive patients who have been “lost-to-follow up”.
+ Work with Bartlett case managers, navigators, and the BCHD linkage-to-care team to contact patients “lost-to-follow up” for re-engagement in care.
+ Point of contact for re-engagement in care collaborative activities with the Johns Hopkins Emergency Department, JHH, and external clinics, Johns Hopkins inpatient teams, and other organizations where people living with HIV but not in care receive services.
_General Nurse activities_
+ Provide primary nursing care for patients.
+ Facilitate communication among the clinical care team of the provider, case manager, and patient.
+ Answer calls from patients requesting emergency appointments. Independently determine the need for medical assessment and advise clerical staff to schedule appropriately.
+ Provide/direct outside agencies to obtain authorization for procedures for HIV-infected patients receiving primary care.
+ Independently assess laboratory reports for abnormalities and implement an intervention plan to resolve the problem.
+ Provide clinical triage during the clinic. Clinically assess the need for diagnostic testing prior to being evaluated by the medical clinician.
+ Assist in assessing and triaging acutely ill patients.
+ Arrange for patient hospitalizations by coordinating the interaction between the attending physician, medical shift coordinator, Infectious Diseases Fellow, and inpatient unit.
+ Conduct educational sessions, individual or group, addressing HIV treatment and management issues as outlined in clinical protocols.
+ Administrative duties.
+ Maintain current knowledge of all JHH and JHU policies and procedures related to unit operation. Assume responsibility for appropriate application and compliance.
+ Attend treatment team and staff meetings.
+ Facilitate HIV education through presentations and discussions regarding specific patients.
_Special Knowledge, Skills, and Abilities_
+ In-depth knowledge of the clinical manifestations and management of HIV infection.
+ Adept at the clinical assessment of patients living with and at risk for HIV infection.
+ Ability to take independent action to care for patients who are acutely ill, awaiting provider evaluation.
+ Strong interpersonal skills required to interact with health care providers and staff from many disciplines.
+ Knowledge of lesbian/gay/gender identity issues and resources.
+ Able to work with culturally diverse patient, staff, and community populations.
+ Strong organizational skills with attention to detail and follow-through.
+ Self-starter with the ability to work with minimal supervision.
+ Strong leadership potential.
Minimum Qualifications
+ Individual must be a registered nurse, licensed in the State of Maryland or the state where practicing.
+ Bachelor's Degree in nursing or related discipline required.
+ Require a minimum of two years of related nursing experience, with additional experience preferred.
+ Additional education may substitute for required experience and additional related experience may substitute for required education beyond HS Diploma/Graduation Equivalent. to the extent permitted by JHU equivalency formula.

Starting Salary Range: $75,100 - $131,700 Annually ($80,000 targeted; Commensurate w/exp.)
