    + JOB REQUIREMENTS

    + Education

    + Registered nurse with current and unrestricted license in the state of Arkansas. A bachelor’s degree is preferred but not required.

    + Experience

    + Management experience preferred, role experience will be considered in lieu of management experience. Documented experience in practice area.

    + Normal hospital environment. Occasional exposure to fumes, odors, biological and electrical hazards. Normal/corrected eyesight with close eye work. Hearing of normal and soft tones. Uses computer, telephone, copier, fax, scanner and adding machine. Lifting up to 20 lbs, carrying up to 20 lbs, pushing and pulling up to 50 lbs. Will be extended of sitting and or considerable walking throughout clinical areas and departments.

    + JOB SUMMARY

    + This is a working/supervisory position responsible for supervision and coordination of admission interview, medication reconciliation, and discharge process and staff development. The position must possess excellent communication skills and function at a high level as a member of the collaborative team. The role of this position is pivotal in coordinating the care of the patient pre and post discharge to facilitate an efficient and effective acute care stay, and coordinate accurate information to the next provider(s) of care to minimize readmissions. This position is required to utilize independent judgment. Ensures compliance with guidelines, institutional policies and procedures, and other information to determine appropriate transfers and admissions into Medical Center. Facilitates care during hospitalization of the assigned case load by coordinating and monitoring resources to achieve optimal clinical and financial outcomes, utilization and discharge. Coordinates with Patient Transfer Center to ensure patient flow is achieved. Responsible for education to medical staff and hospital staff regarding functions related to admission and discharge duties. Responsible for meeting department goals, ensuring appropriate staff in place to produce quality work and meeting budget for these areas. Routinely assigns and responsibly directs staff.
