BAYADA @ Bayhealth is hiring a compassionate **Home Health Admissions RN** to join our team, providing one-on-one care (Admissions, Assessments, etc) to adult and geriatric clients throughout **Sussex County.**

     

    As an Admissions RN, you’ll be the first point of contact for our new clients—conducting start-of-care visits, comprehensive assessments, and building strong clinical foundations for ongoing care in the comfort of patients’ homes.

    Responsibilities:

    + Perform start-of-care visits and comprehensive assessments (including OASIS)

    + Develop and initiate individualized care plans

    + Educate patients and families on the care process and expectations

    + Collaborate with a supportive interdisciplinary team

    Requirements:

    + Active RN license in Delaware

    + At least one year of clinical experience

    + Strong assessment and documentation skills

    + Reliable transportation and ability to travel within Sussex County
