Job Summary and Qualifications
It is the responsibility of the Remote TCT Oncology Pharmacy Specialist to provide comprehensive clinical pharmaceutical care. The Remote TCT Oncology Pharmacy Specialist will perform and document consults, clinical interventions, and medication therapy management to demonstrate efficacious, cost-effective pharmaceutical care. This pharmacist will also assist with medication management through the development of drug monographs, medication use evaluations, staff education, nonformulary requests, and/or high cost approvals as needed. This pharmacist will assist with TCT policy and procedure review. This pharmacist position will work remotely but will still coordinate daily with the pharmacy and TCT multidisciplinary teams to optimize patient care. It is the expectation that Remote TCT Oncology Pharmacy Specialists will obtain and maintain board certification within two years of hire.
+ Active Texas state pharmacist license in good standing.
+ Successfully completed ASHP PGY2 residency and/or four years of equivalent clinical experience
+ Will obtain and maintain Certification in related area of specialty within 2 years, if not already certified (if the specific clinical specialty certification is not available at the time of hire then the certification must be obtained within two years of specialty certification testing availability)
+ Interpersonal skills and empathetic communication abilities to successfully interact with patients, physicians, and other healthcare professionals.
In this role, you will:
+ Provides TCT clinical pharmacy services and drug information to patients, physicians, nurses, and other pharmacists.
+ Completes clinical activities including profile review, medication therapy management (e.g. antimicrobial stewardship, appropriate prophylaxis, vaccine management, disease state management), order review, and consults.
+ Provides evidence-based recommendations to TCT multidisciplinary team.
+ Completes chemotherapy and supportive care order review for accuracy, safety, and optimal dosing
+ Assists with treatment planning and development of order sets/protocols for TCT patients
+ Works closely with TCT multidisciplinary team to optimize medication therapy management
+ Develops clinically appropriate and cost-effective TCT drug therapy protocols and policies
+ Completes TCT nonformulary requests and/or high cost approvals
+ Serves as liaison between pharmacy team and TCT service line
+ Provides TCT patient counseling and chemotherapy education
+ Assists in TCT medication formulary management and compliance, including conducting medication use evaluations
+ Documents activities promptly and accurately
+ Communicates effectively with multidisciplinary and pharmacy teams. Uses communication tools provided to optimize timely communication with the multidisciplinary team
