Job Details Our Nurse Transition Coach/ Quality Management Coordinator (RN) will: Oversee management and navigate a patient’s care to seamlessly transition a patient back to their home or community setting Coordinate a patients’ post-acute plans, ensures follow up appointments with community health care providers are set and treatment (disease specific) bundles are met prior to discharge Facilitate conversations between interdisciplinary Care Teams (nursing, MMC, social work, hospitalists, primary care physicians, home care services, and additional health care providers) and expedite client services referrals Ensure patient/family: Are educated/provided with specific instructions about their condition, treatments, procedures, medications Follow-up phone calls post discharge are completed within 24 – 48 hours Perform related duties as assigned by the Assistant Vice President for Quality Management. What you’ll bring: Passion for Quality Improvement and Education 4+ Years of RN experience working in an acute care hospital Experience in Quality/Performance Improvement preferred Skilled in the use of electronic medical record (EMR), MS Office, MS Excel, & MS PowerPoint Education: Graduate of an accredited School of Nursing required Bachelor of Science in Nursing (BSN) degree required Licensure: New York Registered Nurse (RN) License & Registration Certification: CPHQ certification preferred Salary Range USD $52.31 - USD $60.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
