In this role, you will primarily support West Cuyahoga, Lorain and various west side location counties.
As a Home Health Registered Nurse, you will be responsible for providing high quality nursing care to patients in the comfort of their own homes. This position will give you the opportunity to provide exceptional care and show compassion to patients residing on the west side of Cleveland and surrounding areas. In this role, you will provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. After seeing patients, Home Health RNs work remotely to complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules.
Cleveland Clinic Home Care provides a generous in-depth orientation that is tailored to each individual and their experience level. We provide our nurses with many professional development resources, including furthering education and online/in-person courses to enhance your skills and eligibility for student loan repayment.
A caregiver in this position works 5-8 hour shifts, 9:00am to 5:00pm. Caregivers are required to work 1 winter and 1 summer holiday and every 4th weekend, as well as 1 on-call requirement per week.
A caregiver who excels in this role will:
+ Provide direct nursing care.
+ Perform accurate and complete admission assessments and reassessments within a specified time frame.
+ Assess patients for safety risks, abuse and neglect.
+ Serve as a primary coordinator of all disciplines for well-coordinated patient care.
+ Assess, document, prioritize and manage patients' pain.
+ Escalate changes in patient condition to patient's physician as appropriate.
Minimum qualifications for the ideal future caregiver include:
+ Graduate from an accredited school of professional nursing.
+ Current state licensure as a Registered Nurse (RN).
+ Basic Life Support (BLS) certification through the American Heart Association (AHA)
+ One year of clinical experience as an RN in an acute med/surg setting.
+ Valid Ohio driver's license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
+ Minimum one year current clinical experience as a registered nurse.
+ Minimum one year experience in an acute medical-surgical or home care setting.
Preferred qualifications for the ideal future caregiver include:
+ Bachelor of science in nursing (BSN)
+ Experience working in homecare

Physical Requirements:
+ Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
+ Must be able to read, write and communicate with patients.
+ Must have correctable vision and correctable hearing to be able to hear normal conversations. Must be able to work in settings with infectious diseases.
+ Must present a professional appearance at all times and attend staff and agency meetings, as requested.
+ Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
Personal Protective Equipment:
+ Follows standard precautions using personal protective equipment as required.
