+ Shift information: 8 am – 4 pm (Monday – Friday)
+ **Clients:** Older adults (60+) that do not qualify for home-based chore worker services through Medicaid or those that have not been successful with a community based chore worker that are already enrolled in CSS services at Independence Center
+ **Location:** Community based – this person will work with clients in the community and in their homes
+ Ideal Skill Set: willingness to take action, collaborative skills, initiate and follow through on tasks to completion
+ **Example of a typical Day:** working with 3-4 members in their home, following set goals and providing education and skill building related to maintaining a safe home for members to age in place. Following up with the case worker for any needs that are in the community, alerting treatment team of any challenges they encounter or needs they are unable to address
Overview
**BJC Behavioral Health** is a community health center that provides and coordinates behavioral health services for more than 8,000 seriously mentally ill adults and seriously emotionally disturbed children in St. Louis City, St. Louis County, St. François, Iron and Washington counties. As an Administrative Agent of the Missouri Department of Mental Health (DMH), BJC Behavioral Health serves as a major point of entry for people eligible for mental health services funded by DMH and is responsible for serving as gatekeeper to the public mental health system.

Role Purpose
Responsible for providing essential In-home based supports specifically designed to help adults living with persistent mental illness develop skills to maintain a clean, safe home despite physical limitations. This role addresses barriers to independence through education and modeling of independent living skills while coordinating access to comprehensive physical, environmental, behavioral health and in-home services. This role addresses the critical need for a more robust and patient centered chore service delivery system to ensure the well-being and independence of a vulnerable population.
Responsibilities
+ Completes timely documentation of services that clearly describe the need for the service, the intervention provided, the relationship to the treatment plan, the provider of the service, the date, the actual time and setting of the service, and the individual's response to the service.
+ Provides in home support services to complex clients to include clients on care plan, using treatment plan interventions that result in positive outcomes, based on individual strengths and needs; case load sizes will vary.
+ Completes specialized screening tools to identify and improve areas of need, and develop plan to improve housing skills through modeling and instruction in areas of meal preparation, laundry, home hygiene, organization and other areas of independent living skills.
+ Confers with treatment team to obtain additional information, suggest revisions in treatment plan and to integrate community support, and/or occupational therapy treatment with other aspects of clients' care.
+ BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
