The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. The NCSB is seeking a Case Manager IV to serve as the agency liaison to authorize and monitor acute care, crisis stabilization units (CSU) and indigent admissions while coordinating discharge planning. Provides discharge planning to consumers currently hospitalized in DBHDS State Hospitals. Completes and follows IDAPP consumers in the community on a limited basis in collaboration with other providers.
Department Hiring Salary Range: $50,624-58,217
Essential Functions
Essential functions include but are not limited to:
+ Authorize short-term local acute care and crisis stabilization unit (CSU) bed days.
+ Provide discharge planning to consumers in local acute care, CSU and DBHDS State hospitals.
+ Monitor provision of treatment throughout the hospitalization or stay to ensure utilization of effective treatment modalities. Establishes and maintains collaborative relationships and ongoing communication with providers of inpatient psychiatric acute care and crisis stabilization units.
+ Serves as a NCSB agency liaison for local acute care, CSU and DBHDS State hospital staff and care coordinators in developing appropriate discharge plans.
+ Collaborates with local acute care, CSU and DBHDS State hospital staff and community providers to identify and overcome barriers to timely and effective discharge planning.
+ Coordinate case reviews with the Program Supervisor, Prescription Team, local acute care, CSU's, DBHDS State Hospital Treatment Team and/ or Community Care Providers as appropriate.
+ Links consumers to community resources, services and supports specified in discharge plan.
+ Completes IDAPP plans for consumers hospitalized in DBHDS State hospitals.
+ Identify systems issues which may contribute to an increased length of stay.
+ May manage a caseload.
+ Maintain accurate clinical, statistical and financial records in accordance with Norfolk CSB standards.
Education/Experience
Bachelor's degree in a human services field.
Considerable experience in a human services field related to the area of assignment. Some assignments may also require specific experience working with various types of client populations; master's degree preferred in some areas.
Preferred Experience:
Experience in quality assurance, discharge planning, case management and working with adults with serious mental illness and co-occurring disorders.
Additional Information & Requirements
+ Valid Driver's License may be required depending on assignment.
+ Certification in basic first aid and CPR, depending on area of assignment.

+ Retirement
If you are hired October 5, 2010, or after to a position with City of Norfolk Retirement System benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
If you are hired January 1, 2022, or after to a position with Virginia Retirement System (VRS) benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
+ The Tuition Assistance Program is established to encourage employees toward continued self-development and education. Permanent full-time and permanent part-time classified, unclassified permanent, full-time special projects, and constitutional employees who have completed six months continuous service will be eligible to apply. The applicant’s school of enrollment must be an accredited institution.
