    Job Summary

    Basic Function and Responsibility

    To provide ambulatory clinical pharmacy services (disease management and comprehensive medication review) in collaboration with providers in ambulatory sites affiliated with University of Michigan Hospital and Health Centers. This position will provide clinical support to palliative care services (80%) and adult sickle cell services (20%).

     

    Individual must exhibit a professional and positive image when interacting with patients, faculty and staff. Individual must also adhere to a high standard of personal and professional conduct; possess excellent interpersonal and communication skills and problem solving skills; demonstrated ability to interact with a diverse population. Candidates must be able to work effectively with a multidisciplinary team, handling multiple responsibilities in a fast-paced environment.

    Responsibilities*

    Characteristic Duties and Responsibilities

    + Provide pharmaceutical care by: participating in patient care with the medical staff during outpatient clinic visits; designing, implementing and monitoring patient-specific pharmacotherapy; promoting safe, evidence-based, and cost-effective use of drug therapy; developing and fostering positive collaboration with providers and clinic staff; documenting pharmaceutical care activities and outcomes.

    + Assist in the development and implementation of quality improvement processes to monitor the effectiveness and quality of pharmacists? services.

    + Participate in the training of General Pharmacy Practice Residents, Ambulatory Care Specialty Residents, and Pharmacy Students.

    + Conduct or participate in drug use projects, cost reduction/management initiatives, and quality improvement activities consistent with the Health System?s needs and objectives.

    + Individual or collaborative scholarly/research activity is encouraged.

     

    Supervision Received

     

    Supervision is received from the Director Population Health Pharmacy and Community Partnerships.

     

    Supervision Exercised

     

    Functional supervision is exercised over Pharmacy Residents and Students.

    Required Qualifications*

    + A Doctor of Pharmacy degree.

    + A certificate of completion from a General Practice residency is necessary.

    + A certificate of completion from a specialty residency in palliative care is necessary.

    + Experience in ambulatory clinical pharmacy practice is necessary.

    + Licensure as a Pharmacist by the State of Michigan is necessary.

    + Possession of a state controlled substance license is necessary.

    + Demonstrated ability to collaborate clinical care ideas across multiple patient care services and to communicate effectively as a patient advocate.

    + Demonstrates a commitment to carrying out professional responsibilities, adherence to ethical principles, and sensitivity to diversity with a commitment to service excellence and Patient Family Centered Care.

     

    Work Schedule

     

    Anticipated Start Date: August 2025

     

    Hours/Week: 40 hours, full-time

    Background Screening

     

    Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings. Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
