What You’ll Do
+ Conduct initial and follow-up assessments to evaluate patients in their home setting
+ Create and implement individualized, evidence-based care plans
+ Document patient progress and adjust plans as needed
+ Instruct patients and caregivers on therapy goals, exercises, and equipment
+ Work closely with nurses, social workers, and other clinicians to ensure coordinated care
+ Supervise Physical Therapy Assistants in accordance with state regulations
Qualifications
+ Current (or eligible) Maine licensure from the Board of Examiners in Physical Therapy
+ At least 1 year of professional experience as a Physical Therapist
+ Valid Maine driver’s license with a clean driving record
+ Ability to work independently and exercise sound clinical judgment in a home setting
