Job Description:
The primary role is to support Triage and Transitions RNs (Registered Nurse). This individual will focus on ensuring consistent, reliable, and high quality clinic processes, protocols, workflows, and training for RNs and clinic teams. This role will strongly collaborate with the Practice Administrators, Clinic Managers and partner with Process Improvement, Ambulatory Informatics, Pharmacy and other system resources.
_This position is a hybrid position which will require both remote and in-person/on-site visits. If you are currently an internal caregiver for Intermountain Health, you must reside in either CO or MT close to an Intermountain care site. If you are not a current caregiver for Intermountain Health and are applying externally, you must currently reside in Colorado or Montana near an Intermountain Health care site._
ESSENTIAL DUTIES AND RESPONSIBILITIES
Responsible for supervision, development and coaching of Triage and Transitions RNs
Responsible for evaluating outcomes of transitional care management services and telephonic triage services provided by RN team
Coach providers, MAs and other associates on building integrated clinical care teams
Build and effectively maintain relationships with practice leadership and key clinical influencers to result in practice transformation
Plan, develop and implement strategies to address population health needs for primary care clinics within the region
Protect patients and employees by developing and interpreting policies and protocols for telephonic triage, transitions of care, and other population health initiatives.
Promotes mission, vision, and values of Intermountain Health, and abides by service behavior standards.
Performs other duties as assigned.
EDUCATION
Required: Graduation from an accredited School of Nursing is required.
BSN required.
**CERTIFICATION, REGISTRATION, LICENSE** (*indicates primary source verification requirement)
_Required:_
+ Current compact (multi-state) license as a Registered Nurse in the state of practice is required.
+ Current Basic Life Support for Healthcare Providers (BLS-HCP) course completion card from the American Heart Association is required
EXPERIENCE
_Required_ : Experience working as a Registered Nurse at least 3 years
_Preferred:_
+ 3+ years of acute care and/or ambulatory care nursing experience is strongly preferred
+ Primary care nursing and/or care management experience preferred. 2+ years of leadership/management including process improvement and change management experience

• Proficient in advocating for MA and RN clinical support roles as they
pertain to primary care patient centered care
• Strong understanding of the value of RN role on an integrated primary
care team
• Interacts with colleagues, team, and all staff in a professional and
collegial manner
• Strong writing, process, critical thinking and communication skills

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$44.33 - $68.42
