    About the Department

     

    The Towers Outpatient pharmacy at Trinity Health Ann Arbor is open 7 days a week, offering various programs and services to help support and discharge patients.

    Position Purpose

    A Community Outpatient Pharmacist provides pharmaceutical care for the outpatient populations according to professional standards and statutory requirements. Directs the functional activities of the outpatient pharmacy technicians, students, and interns. Provides clinical support, medication preparation and distribution in conjunction with other members of the healthcare team to ensure high quality, safe and cost-effective drug therapy to patients. Provides education, consultations, immunizations, and medication therapy management to patients to improve patient outcomes. Engages in professional activities to improve the public's health awareness.

    **Status available:** Full-time

    **Hours:** M-F 7:30a to 6pm, Sat 8am -4pm, Sun 9am-3pm

    REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

    Education

    + Bachelor’s degree in Pharmacy from an accredited College of Pharmacy is the minimum acceptable level of education.

    + A Doctor of Pharmacy degree (PharmD) is strongly preferred.

    Required Skills and Abilities

    + Broad knowledge of medications, compounding, and dispensing.

    + Thorough knowledge of pathophysiology, pharmacokinetics, and therapeutics.

    + Interpersonal skills necessary to communicate effectively with a wide variety of hospital healthcare personnel and diverse patient population

    + General understanding of computers, including general operations and data retrieval

    + Communications skills necessary for clear written and oral communication.

    + Excellent customer service orientation skills necessary in order to deal effectively with various levels of hospital personnel, outside customers and community groups.

    + Training and skill to administer immunizations

    + Maintain a working knowledge of applicable Federal, State, and Local laws and regulations

    What you will do:

    + Interprets and evaluates all prescriber’s medication orders to ensure appropriate drug therapy for each patient in accordance with legal and regulatory policies, rules, and standards.

    + Reviews, maintains, and screens patient medication orders and profiles for allergies, drug-drug interactions, drug-food interactions, contraindications, potential adverse drug reactions, therapeutic duplication, and out of range doses.

    + Communicates with physicians, nurses, and other healthcare providers and patients to intervene on orders that are unclear, incomplete, inappropriate, or could cause drug misadventures and recommends alternative therapy. Intervention activity is documented appropriately.

    + Performs patient medication histories and counseling when necessary or requested. Provides education materials, as required.

    + Consults with and advises physicians and other health care professionals in the development of safe, rational, and cost-effective patient medication care plans.

    + Supports and conducts one's self in a manner consistent with customer service expectations and the Trinity Core Values.

    + Directs and monitors pharmacy technicians, interns, and students in preparing, packaging, labeling, and distributing of medication.

    + Compounds and dispenses basic medications; recognizing that compounding and/or dispensing errors may result in significant patient morbidity and mortality.

    + Initiates, directs, coordinates, and performs specific departmental projects.

    + Participates in teaching programs affiliated with the Department of Pharmacy, and training programs for nursing, physicians, pharmacists, and other allied health professionals.

    + Assists in the training and orientation of new pharmacists, pharmacy technicians and pharmacy interns.

    + Administration of vaccinations to patients following established pharmacy policy.

    + Maintains control and accountability for disposition of Controlled Substances, dangerous or toxic drugs and investigational drugs as required by Federal and State regulations. Properly maintains all legally required records.
