Facility:Dayton Children's - Main CampusDepartment:Integrated CareSchedule:Full timeHours:40Job Details:The Manager of Integrated Care directs the activities of and supervises the RN Care Managers to provide and facilitate care coordination of timely/necessary medical care, discharge planning, utilization management, and community referrals. Develops and implements standard work for all direct reports. These activities, consistent with the mission, strategic plans, and policies of the organization, will be conducted in collaboration with other departments to ensure appropriate and efficient usage of resources while providing quality services to patients and their families throughout the continuum of care. In addition to performing the essential functions listed below, the Manager may also be assigned other duties as required.

Functions as a member of the multidisciplinary health care team to facilitate patient movement through the continuum of care and back into the community. The Integrated Care Manager supports the physician and interdisciplinary team in facilitating patient care with the underlying objective of enhancing the quality of the clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors. As part of the team, makes decisions regarding utilization management and alternative treatment which reflects cost effectiveness. Provides interface with internal departments and external departments and agencies. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. In addition, the care manager helps drive change by identifying areas where performance improvement is needed.

Department Specific Job Details:

Education

+ BSN degree

+ Master's Degree preferred

Experience

+ 7-10 years clinical experience as an RN required

+ Minimum of 5 years recent experience in Utilization, Care Management, and Discharge Planning preferred

+ Minimum of 3 years management experience in a similar department preferred

Certifications

+ Registered Nurse with current Ohio RN license required

+ Current professional certification as a care manager preferred

Education Requirements:

Bachelors: Health Administration (Required), Masters

Certification/License Requirements:

[Lic] RN: Registered Nurse (RN) - Ohio Board of Nursing
