    Description

    Comunilife Inc.is looking for an experiencedProgram Director for its new Social Care Network Services (SCN)program. Under the supervision of the Vice President for Programs, the Director of the Social Care Network (SCN) Services will lead the implementation, delivery, and continuous improvement of services across multiple regional Social Care Networks (SCNs), with a focus on enhancing health equity through coordinated social care. This leadership role will oversee care delivery operations, manage high-performing teams, ensure contract compliance, and serve as a key liaison with internal and external stakeholders, including community-based organizations, health plans, government partners, and healthcare providers. The Director will be instrumental in advancing whole-person care for Medicaid populations in the Bronx, Brooklyn, Manhattan, and Queens, and ensure the successful execution of SCN goals aligned with NYS Department of Health guidelines. The Director will ensure the appropriate delivery of Health Related SocialNeeds (HRSN) services. The Director will have supervisory oversight of the HRSN program staff that include the HRSN Specialist and HRSN Care Coordinator.This position is eligible for Comunillife Inc.'s generous paid time off and benefits package.

    Mon.-Fri. 9am - 5pm

    Essential Duties:

    + Serve as the primary point of contact for enrolled SCNs operations across assigned regions.

    + Oversee Health-Related (HRSN) service care delivery strategy and execution, applying project management best practices to track outcomes and meet performance milestones.

    + Lead cross-sector collaboration with CBOs, managed care organizations (MCOs), government entities, and technology partners.

    + Supervise and support SCN care delivery teams, promoting a culture of excellence, accountability, and continuous learning.

    + Ensure full alignment of budget and resources, including development and oversight of scopes of work, performance targets, and reporting mechanisms.

    + Monitor program compliance, including case reviews, internal audits, and database management to ensure adherence to state and funder requirements.

    + Develop and deliver compelling presentations and reports for internal leadership, governance committees, and external partners.

    + Collaborate with senior leadership to guide strategic planning, innovation, and sustainability of the SCN program.

    + Stay informed on emerging trends in public health, SDoH, and Medicaid policy to inform decision- making and program adaptation.

    + Lead internal and external advocacy efforts, working groups, and committees to elevate the impact of the SCN.

    Qualifications

    Education and Experience Requirement(s)

     

    + Minimum: Bachelor’s Degree in Public Health, Health Administration, Social Work, Public Administration, or a related field. Preferred: Master’s Degree (MPH, MPA, MBA, MSW, or equivalent) from an accredited institution.

    + Minimum of 10 years of experience in nonprofit, public health, or healthcare management.

    + At least 3 years in direct project management and supervisory roles.

    + Strong understanding of the health and human services ecosystem in NYC/NYS and Medicaid programs.

    + Proven ability to manage budgets, oversee contracts, and ensure compliance with complex funder requirements.

    + Demonstrated success in team leadership, program development, and interagency collaboration.

    + Excellent written, oral, and public speaking communication skills.

    + Proficiency in Microsoft Office and familiarity with CRM, EHR, or other case management systems.
