    Shift: Days

     

    Hours Per Week: 40 Hours

     

    Hiring range: $ 23.46 - $30.50 The actual compensation for this position will be determined based on experience and other factors permitted by law.

    Responsibilities:

    + Educates patients/families about disease treatment plan including self-care post discharge, available resources and follow up care.

    + Schedules discharge appointments, using clinical judgment based on medical need and complexity.

    + Collaborates with home care companies as well as inpatient case management.

    + Be knowledgeable with medications to complete medication reconciliation and education with patients.

    + Create, implement and update patient centered care plans.

    + Clear and concise documentation within the EMR

    + Build and maintain professional relationships with the clinic staff and providers to promote the Chronic Care Program.

     

    Complexity of Work: • Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment.

     

    Personal Protective Equipment: • Follows Standard Precautions using personal protective equipment as required.

     

    Required Credential(s): • BLS Provider obtained within 3 Months (90 days) of hire date or job transfer date. • Licensed Practical/Vocational Nurse credentialed from the New York Board of Nursing.Must be to speak, read, write and follow instructions in English
