Monday–Friday | No Weekends or Holidays | Day Shifts Only

• No weekends, holidays, or on-call shifts

• Supportive Monday–Friday schedule with consistent day hours

• Opportunity to build meaningful relationships with patients and families

• Work independently while being backed by a collaborative interdisciplinary team

Your Day-to-Day:

• Provide personal care and companionship

• Take vital signs and report on patient condition

• Travel between patient homes in the Fond du Lac area

• Collaborate closely with the RN and care team

• Support families with empathy and dignity

Qualifications

+ Meet the training requirements of the state of practice

+ Minimum of six months experience including providing personal care

+ At least 18 years of age

+ Complete a minimum of 75 hours of classroom and supervised practical training, 16 of the 75 hours of classroom must be complete prior to a mandatory 16 hours of practical training

+ Possess and maintain current CPR certification

+ Ability to read and follow written instructions and document care given

+ Understands hospice philosophy, comfortable providing specialized care to the terminally ill

+ Ability to work with little direct supervision

+ Ability to handle death/dying

+ Strong Oral and written communication skills

+ Good Organizational skills
