    The Associate Director, Integrated Clinical Pharmacist delivers forward-thinking leadership in the coordination and enhancement of pharmacy services across multiple clinical sites. This position oversees the clinical pharmacist team, drives the optimization of medication therapy management, and ensures the achievement of productivity targets and outcome measures, with a particular emphasis on chronic disease management. Through effective collaboration with multidisciplinary teams, the Associate Director supports the delivery of comprehensive, patient-centered care for vulnerable populations, regardless of health plan coverage, while promoting operational excellence and quality improvement.

    Key Responsibilities:

    Strategic Leadership & Team Management:

    + Provide visionary leadership to a multidisciplinary pharmacy team, fostering a culture of clinical excellence, accountability, and continuous professional development across multiple clinical locations.

    + Evaluate current program strengths, technology assets, clinical capabilities, and operational workflows to identify gaps and opportunities for innovation.

    + Develop phased implementation plans with defined milestones, deliverables, resource requirements, and contingency strategies.

    + Implement robust change management practices to facilitate adoption, address resistance, and ensure sustainability.

    + Develop, implement, and evaluate pharmacy services that align with organizational objectives and evolving healthcare trends.

    Clinical Integration & Patient Care:

    + Direct comprehensive medication therapy management programs, ensuring evidence-based, safe, and cost-effective pharmaceutical care.

    + Oversee execution of clinical protocols in compliance with state regulatory agencies and organizational policies.

    + Facilitate post-discharge medication reconciliation, chronic disease management and patient education.

    + Collaborate with physicians and care teams in regular market meetings to develop and implement individualized patient care strategies.

    Data Analytics & Performance Improvement:

    + Lead the design, implementation, and interpretation of pharmacy-related data analytics initiatives to identify trends, forecast needs, and drive informed decision-making.

    + Develop actionable insights and present data-driven recommendations to the National Director of Pharmacy for discussion.

    + Ensure accuracy and integrity in EMR and pharmacy management system documentation and reporting.

    Quality, Compliance, & Innovation:

    + Maintain oversight of regulatory compliance and accreditation requirements relevant to pharmacy practice.

    + Lead quality improvement initiatives, integrating innovative solutions and best practices to optimize medication therapy and patient safety.

    + Educate providers and staff on medication management, clinical protocols, and emerging pharmaceutical advancements.

     

    Use your skills to make an impact

    Minimum Requirements:

    + Active and unrestricted pharmacist license in the state(s) of practice.

    + Doctor of Pharmacy (Pharm.D.) or Bachelor of Pharmacy

    + Minimum of 10 years of experience as a practicing pharmacist, including at least 5 years in leadership.

    + Demonstrated expertise in pharmacy practice management and clinical integration.

    + Experience in developing and applying data analytics within a healthcare environment.

    + Proficient in MS Office applications (Word, Excel, PowerPoint) and EMR systems.

    + Excellent organizational, analytical, verbal, and written communication skills.

    + Ability to drive change, work collaboratively in interdisciplinary teams, and manage multiple priorities in a fast-paced environment.

    Preferred Qualifications:

    + Completion of a clinical residency in ambulatory care or related discipline.

    + Board certification in geriatric or ambulatory care pharmacy.

    + Experience with advanced analytics tools and techniques (e.g., Power BI, SQL).

    + 3+ years of experience in ambulatory care or primary care setting.

    + Proven track record of successful quality improvement and innovative pharmacy program implementation.

    + Multilingual abilities (with preference for English/Spanish).

     

    Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

    Pay Range

    The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

    $150,000 - $206,300 per year

    This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
