Salary Range: $21.00 - $28.00 Hourly
Role and Responsibilities : Responsible for developing and maintaining client relations and intake of direct referrals made to HCR .
Essential Functions
+ Cultivates relationships with physicians and hospitals to facilitate the best Home Care solutions for their patients.
+ Monitors HCR RightFax folders, label faxes and attach electronic documents to patient record in Home Care Home Base (HCHB).
+ Monitors Intake Team email.
+ Produces timely, accurate data entry.
+ Enters orthopedic prescreens into the EMR according to agency protocol.
+ Provides phone support to the intake department and triages calls to Intake staff.
+ Provides excellent customer service to internal and external customers.
+ Participates in department and agency meetings.
+ Other duties as assigned .

Education Requirements
+ Completion of an approved program of Practical Nursing required .
+ Valid/Unencumbered NYS LPN license required.
Qualifications and Requirements
+ A minimum of 4 years of related experience.
+ Exceptional customer service and communication skills required .
+ Proficient computer skills; including solid comfort level navigating within an EMR system
+ Detail oriented and able to work in a fast-paced environment .
Work Environment
The Client Relationship Specialist is primarily in an office setting and may be exposed to outdoor conditions.
The working conditions are classified as sedentary work:
+ Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
