Salary Range: $60,500.00 - $88,000.00 Annually
Role and Responsibilities : The Home Care Coordinator (HCC) is a NYS licensed RN or LPN who participate s in communication and discharge planning during a patient’s transition from acute care to the home care setting. The HCC collaborates with hospitals to prevent patient rehospitalization. Manage referrals and coordinate safe home care services for patients, in accordance with physician . Accountable for developing positive relationships with referral sources
Essential Functions :
+ Cultivates relationships to help physicians and hospitals identify the best home care solution for their patients .
+ Collaborates with the acute care setting's interdisciplinary team to facilitate referrals and resulting in timely sharing of patient information during transitions to and from home care. Interviews patient and family/designated back-up.
+ Uses outreach tools to effectively communicate with referring sources.
+ Invokes the nursing process, identifies qualifiers for home care, provides sufficient overview of both hospital course and identified initial home care need.
+ Develops an initial plan of care for each patient.
+ Obtains insurance authorization and verify physician signing home care orders.
+ Meets agency referral metrics.
+ Ability to clearly articulate an understanding of home health care reimbursement issues.
+ Exhibits a ctive listening skills in order to conduct a comprehensive assessment including physical, psychosocial, and anticipated environmental needs.
+ Communicates with patient's primary care physician to safely transition patient to home care services, and secures follow up appointment within 1 week of hospital d/c. Obtains a copy of the patient’s hospital discharge instructions to facilitate medication reconciliation with physician.
+ Provides accurate and timely documentation via the EMR to facilitate the referral process. Documents according to facility/agency regulatory guidelines.
+ Functions as a team with fellow HC Cs , agencies, and other HCR departments.
+ Assists others as needed, displaying flexibility and effective problem-solving techniques in response to fluctuating patient volume and vacation coverage.
+ Effectively communicates with internal business partners to ensure a safe, effective home care plan for all HCR patients.
+ Attends required HCR meetings, in - services , and functions.
+ Conducts educational sessions with physicians and hospitals .
+ Develops and maintains working knowledge regarding all of HCR programs and services.
+ Works in conjunction with community outreach staff , as needed.
+ Demonstrates continuous professional development and participation in self and peer evaluation.
+ Other duties as assigned .

Education Requirements
+ Current, valid nursing license (LPN or RN) issued by NYS Department of Education.
+ Associate's Degree in nursing is required.
+ Bachelor’s Degree in nursing is preferred.
Qualifications and Requirements
+ Current, valid RN or LPN license from NYS Department of Education required .
+ Two years of community health/home care, hospital, or equivalent Home Care Coordinator experience .
+ Excellent interpersonal communication skills .
Work Environment
The Home Care Coordinator is primarily in an office setting and may be exposed to outdoor conditions.
The working conditions are classified as light work:
+ Light work - Exerting up to 20 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Light work involves sitting most of the time.
