    Summary

    + The Social Worker provides medical social casework to a wide array of customers ranging from newborn to the advanced geriatric patient. The main focus of the social worker is to assist in meeting the psychosocial/continuum of care needs of the patient/resident/tenant and family to ensure optimum functioning. In addition to direct care services, the social worker shall have duties of consultation, education, policy and program planning, performance improvement tasks, advocacy and liaison with the community. This position also provides services in all Sioux Center Health departments as needs are identified.

    General Hours of Work

    + M-F Days, Rotating On Call Schedule for some nights and weekends

    Education and/or Experience

    + Bachelor’s degree in Social Work is required. Work experience or internship in a health care or medical setting preferred. Bi-lingual experience preferred but not required.

    Certificate, Licenses, Registrations

    + Iowa license as a certified social worker preferred.

    Essential Functions

    + Responsible to plan, organize and implement social work services to patients/residents/tenants and families across the continuum of care; assuring that the medically related emotional and social needs are addressed on an individual basis.

    + Interviews and assesses patients/residents/tenants and family members to obtain information about home environment, family systems, health history, strengths, personality traits, etc.

    + Assists patients/residents/tenants and families to understand, accept, and follow medical recommendations with emphasis on assisting patient/resident/tenant to be medically compliant.

    + Performs a variety of services such as advising on social problems, arranging for admission, discharge or post-operative care at home or in institutions, assisting patients/families needing placements into assisted living or nursing homes, and alleviates anxieties and fears.

    + Utilize resources to build up support and resiliency factors for each patient/resident/tenant who is identified as vulnerable due to a lack of resources, support, or identified as an ethnic minority.

    + Prepares and updates records for each patient/resident/tenant. Prepare required documentation of each case. Maintains records, reports as required.

    + Serves as the social work representative on multi-disciplinary team. Works collaboratively with other disciplines in providing coordinated services.

    + Performs assessments and follows protocols most appropriate for patients/residents/tenants and caregiver’s needs.

    + Evaluate data gathered in terms of the medical plan of treatment, available social service programs, financial counseling, and plan a pertinent therapy program that will provide maximum benefits and safety for each patient/resident/tenant.

    + Evaluates and assists patient/family and agency staff in utilizing community resources Provide social service consultation and in-services as requested.

    + Performs other duties and responsibilities as assigned.

    In addition to the above essential functions, the following are specific to the specified areas: Long Term Care

    + Assists staff in meeting the needs of the residents and in practicing routines based on the residents wants, and desires.

    + Assists the team to create an interest-based activity plan as scheduled.

    + Makes written and oral reports/recommendations concerning activities of the household.

    + Must have or will be required to obtain a valid chauffeur’s driver’s license.

    Hospice/Palliative Care

    + Completion and documentation of psychosocial assessment of the patient and families response to illness including evaluation of strengths, coping skills, supports, characteristics of family, emotional, financial, legal, and environmental needs.

    + Admission and discharge planning with hospice program, to help the patient and relevant other to understand the nature of the problem and its importance, facilitate their adaptations to their new roles, and help arrange for care. Serves as a liaison between patient and community resources, necessitating keeping current on federal and state regulations that may affect patients. Assists patients and families with questions regarding advance directives for health care. Serves as an advocate for patient’s rights including dignity, confidentiality, and access to care within and outside the hospital.

    + Keeps precise, informative, current, and legible records of all patients receiving social work services in accordance with agency policies and procedures, certified home care regulations and third party payer requirements.

    + Prepares care plans and genograms as needed.

    + Coordinates articles for Hospice newsletters, PR for Hospice as needed, and presentations for Hospice training; including Annual Hospice Memorial Service.

    + Participates in hospice interdisciplinary group conferences identifying social problems and assisting team members in understanding the relationship between these social problems and medical situations for enhanced care of the terminally ill patient and family.

    + Develops and conducts staff, individual, family and group therapies and/or education as necessary; including support groups.

    + Provide Home Health and Hospice consultation/support services to outpatients at physician offices and facilities.

    + Develops and implements the Bereavement Program policies and procedure and coordinates bereavement before death and follow-up care after the death of the Hospice patient including: risk assessment, bereavement mailings, bereavement visits, counseling, and documentation of each.
