Are you passionate about making a difference in patients' lives as they transition from hospital to home care or hospice services?
Trinity Health At Home is seeking a dedicated Home Care/Hospice Coordinator to join our team and help ensure seamless, compassionate care for our clients.
Key Responsibilities
+ Facilitate referrals from acute care settings to homecare/hospice services.
+ Educate hospital staff and physicians on available services.
+ Coordinate care plans and obtain physician orders.
+ Assess client needs and recommend appropriate services.
+ Maintain communication with healthcare teams, clients, and families.
+ Participate in quality improvement and business development initiatives.
+ Meet sales targets and build referral relationships with providers.
Qualifications
+ Bachelor’s degree or equivalent experience required.
+ Clinical education and licensure preferred.
+ Minimum 2 years’ experience in acute care, customer service, or sales/marketing.
+ Valid driver’s license and reliable transportation.
+ Strong communication and organizational skills.
+ Commitment to ethical standards and continuous improvement.
