    Job Profile** **Summary

    •This role focuses on providing medical and clinical support services to patients. In addition, this role focuses on performing the following Community Health duties: Delivers training, facilitates sessions, and educates the community on a variety of clinical topics. A management role that supervises employees focusing on tactical, operational activities within a specified area, with the majority of time spent overseeing area of responsibility, planning, prioritizing and/or directing the responsibilities of employees. Goal achievement is typically accomplished through performance of direct and/or indirect reports. A role that supervises all levels of employees. Responsibilities that typically include: Setting goals and objectives for team members for achievement of operational results, problems faced may be difficult but typically are not complex, and ensures policies, practices and procedures are understood and followed by direct reports, customers and stakeholders.

     

    Job Overview

     

    This position is responsible for the coordination and management of the clinical and operational aspects of Community Health Programs, such as WIC or Healthy Families Program. The role ensures program integrity, staff supervision, compliance with regulations, quality improvement, and positive outcomes for participating families.

    Job Description

    Minimum Qualifications** **:

    1. Bachelor’s degree in Health or Human Services.

    2. Two (2) years of experience in a supervisory capacity in Community Health or community-based care.

    Preferred Qualifications** **:

    1. Master’s degree in Health or Human Services.

    2. Prior management and administrative experience.

    **Duties and Responsibilities** **:** The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

    1. Responsible for the day-to-day operational activities of participant services that involve the planning, implementation, coordination, and evaluation of participant care services.

    2. Provides program consultation and staff development.

    3. Directs and encourages the growth of assigned Community Health Programs. Participates in marketing the program.

    4. Participates in establishing administrative policies and practices.

    5. Ensures all required policies and agreements are carried out.

    6. Ensures that all necessary committees are formed and committee meetings held.

    7. Implements and evaluates the program's compliance with required regulations, standards, and/or funding requirements.

    8. Develops and ensures standards which ensure safe and effective services to participants and families are met.

    9. Participates in coordinating programs that enhance community collaboration and community coalition-building.

    10. Responsible for adherence to established procedures for recruitment, selection, promotion, and termination of personnel.

    11. In collaboration with Administrator and Finance Director, develops an annual operating budget and capital expenditure plan for services.

    12. Prepares and submits reports as required.

    13. Assists in seeking funding for program expansion, program support, and special projects.

    14. Responsible for QA/QI for the Program.

    15. Provides guidance and overall supervision of: Home Visitors, Supervisors, Administrative Assistants, Consultants, and personnel.

    16. Plans and coordinates educational offerings.

    17. Reviews and revises as necessary.

    18. Ensures that manuals, guides, and other informational materials for use by staff are developed.

    19. Determines a staffing pattern which accomplishes stated objectives.

    20. Monitors staff performance and supports ongoing professional development, coaching, and retention.

    21. Maintains strong partnerships with state agencies, community-based organizations, and funders to enhance program effectiveness.

    22. Represents the program at local and statewide meetings, coalitions, or conferences as appropriate.

    Physical Requirements:

    1. Occasionally lift and/or move up to 25 lbs.

    2. This is largely a sedentary role, which involves sitting most of the time, but may involve movements such as walking, standing, reaching, ascending/descending stairs and operating office equipment.

    3. Frequently required to speak, hear, communicate and exchange information.

    4. Ability to see and read computer displays, read fine print, and/or normal type size print and distinguish letters, numbers and symbols.

    Skills & Abilities:

    1. Ability to plan, organize, and direct the activities of others.

    2. Ability to work cooperatively and communicate effectively with others.

    3. Demonstrated leadership and decision-making capabilities.

    4. Sensitivity to the needs of the participant population.

    5. Ability to articulate program goals and policies to participants, healthcare providers and community agencies and vendors.

    6. Ability to work well with management information systems.

    7. Knowledge of child development, parenting strategies, and family systems.

    8. Experience with funding and grants.
