JOB SUMMARY
The RN – Oncology Navigator Care Coordinator works closely with medical oncologist(s) and APP(s) to support clinic workflows and optimize patient experience. The RN – Oncology Navigator Care Coordinator is responsible for managing the roles of Navigation, Survivorship Coordinator and Principal Care Management as it relates to patients cared for in Whidbey’s cancer program. The RN – Oncology Navigator Care Coordinator manages incoming referrals and care coordination, monitors, records and reports symptoms or changes in a patient’s conditions. The RN – Oncology Navigator Care Coordinator requires excellent patient teaching, assessment, communication, problem solving and symptom management skills; can work independently.
The Medical Oncology Practice RN works closely with medical oncologist(s) and APP(s) to support clinic workflows and optimize patient experience. This role includes provider support, oversight and support to MA staff, coordinating patient care, managing outbound referrals, overseeing genetic testing, and collaborating with tertiary centers. The RN plays a key part in ensuring safe, timely, and effective care through strong communication, clinical judgment, and team collaboration.
The RN – Oncology Navigator Care Coordinator and the Medical Oncology Practice RN are expected to cross-train and provide backup support for one another as needed.
Principle Functions include the following, other duties may be assigned:
Oncology Navigator Care Coordinator:
Oral Chemotherapy
+ Provide oral chemotherapy education.
+ Facilitates specialty pharmacy refills, funding with grants, Patient Assistance Programs, refills, prior authorizations, and re-enrolments.
+ Coordinates oral chemotherapy prescriptions, patient education, and financial assistance in collaboration with the patient and specialty pharmacy, including obtaining prior authorizations, managing denial appeals, and facilitating re-enrollments.
+ Assists patients in applying for local, state, or federal financial assistance programs, providing necessary documentation and securing required signatures.
+ Manages REMS/Celgene portal access for patient and provider surveys, as well as obtaining monthly authorization numbers for medication refills.
+ Provides telephone and in person triage support, symptom management and education for patients on oral chemotherapies, schedule lab draws and results, and coordinate plan of care with ordering provider.
Provides Oncology Navigation and Coordination of Care:
+ Provide community wide navigation support to Whidbey Health patients who have received a positive biopsy result.
+ Facilitate/triage new oncology and hematology referrals to WH.
+ Facilitates communication with referring providers for coordination of care.
+ In the absence of Medical Oncology Practice RN facilitates coordination of concurrent radiation/chemotherapy regimens, specialty consults and outside providers that may be involved with an oncology patients care team.
+ Facilitates self-referral mammogram results to patient and appropriate provider.
+ Provides navigation for patients needing diagnostic mammograms.
+ Participates in breast cancer surgical consultation appointments in collaboration with the breast surgical team.
+ Maintains Community Cancer Resource list.
+ Collaborates with WH hospitalists and Emergency Department providers regarding patient discharges for individuals recently diagnosed with cancer to facilitate seamless navigation and follow-up care.
Coordination of Care with Tertiary Centers:
In the absence of, or in collaboration with, the Medical Oncology Practice Registered Nurse:
+ Act as a liaison, navigator and care coordinator between tertiary centers and clinical teams to ensure seamless continuity of patient care.
Commission on Cancer (CoC) Accreditation:
Participates in meeting Standards as set forth by the Commission on Cancer (CoC) for Accreditation:
+ Standard 4.8 Survivorship Program Survivorship Coordinator
+ Creates and Manages Survivorship care plans and treatment summaries.
+ Identifies eligible patients, track treatments and prompt provider when survivorship office visit is due.
+ Participates in various aspects of the accreditation process to successfully meet Standards. This includes attendance at committee meetings. In absence of, or in coordination with, the Medical Oncology Practice RN, this may include tracking referrals for genetic counseling and clinical trials.
+ Works closely with other departments to track psychosocial distress screening, nutrition, and rehabilitation.
+ Participate in Tumor Board meetings.
+ Organizes and carries out community engagement activities, primarily cancer prevention, screening activities and community education.
Provides education opportunities for oncology clinical staff when care coordination is the goal:
+ Includes direct training, email alerts or requests for electronic module training. Maintain the Cancer Resources list by adding to or updating it when appropriate.
+ In partnership with Medical Oncology Practice and triage RNs, manages Principal Care Management using proprietary database and modes of communication.
Other duties
+ Collaborate with leadership and colleagues to assess, revise, and develop processes aimed at enhancing service quality and operational efficiency.
+ Provide clinical support/guidance to PFS team.
+ May be assigned other duties per program needs.
Medical Oncology Practice RN:
+ Participate in daily huddle with provider and infusion RNs
+ Manage daily workload tasks for providers
+ Communicate lab results and negative diagnostic results to patients PRN
+ Assist oncology providers with patient care coordination before, during and after visits
+ Facilitates peer to peer contact for providers with external providers
Delegation and Training for Medical Assistants:
+ Delegates and trains for task prioritization, chart prep and access to necessary resources and information.
+ Serve as back up for checking in patients arriving for office visit.
Manage and Track Outgoing Referrals and Services:
+ Lab test order entry/coordination with provider and lab for patients who are not on active treatment
+ (duplicate)
+ Provide results to outside medical programs PRN
+ Confirm receipt of orders for outgoing referrals for various procedures/diagnostics, ensure all pertinent records have been received and patient is scheduled
+ Coordination of BMBX and BX referrals to ensure timely scheduling and secure pathology results
+ Coordination of concurrent radiation/chemotherapy regimens, specialty consults and outside providers that may be involved with an oncology patients care team.
Genetic Testing Referrals:
+ Manage and track genetic resting referrals, results, and documentation, ensuring follow-up and integration into the patient care plan.
+ Clinic liaison for representatives
+ Coordination of Care with Tertiary Centers and Specialist Consults
+ Liaison between tertiary centers and clinical teams to ensure seamless continuity of patient care
+ Track and monitor referrals for second opinions
+ Track, monitor and coordinate care for patients referred for clinical trial evaluation/participation
+ Track and coordinate for patients who are preparing for, or being discharged s/p transplant
+ Communicate pertinent information with infusion team
+ Track and secure consult notes for patients referred out
+ Communicate summary of findings to oncology providers
+ Ensure communication is scanned into EMR and available for chart prep
Treatment Clearance:
+ Obtain clearance for treatment restarts when treatment is held for such things as dental clearance, surgery, cardiovascular complications etc., when external provider input is required.
+ Track and manage dental clearance requests
Navigator RN Support:
+ Provide coverage for navigator RN if an urgent referral is pending and navigator is out of the office
+ Provide modified support to the navigator role if navigator is out of the office for more than 3 consecutive days (phone coverages, coordination of referrals)
Oral Chemotherapy:
+ Provides oversite, symptom management and patient education for patients receiving oral chemotherapy:
+ Provides oral chemotherapy education and signing of consents, when NP is unavailable.
+ Facilitates specialty pharmacy refills, funding with grants, Patient Assistance Programs, refills, prior authorizations, and re-enrolments.
+ Coordinates oral chemotherapy prescriptions, patient education, and financial assistance in collaboration with the patient and specialty pharmacy, including obtaining prior authorizations, managing denial appeals, and facilitating re-enrollments.

Wage Range: $37.450 - $70.012
