    Summary The physical therapist's primary function is direct patient care in the Physical Therapy Section of the Physical Medicine and Rehabilitation Service (PM&RS) at the VA Ann Arbor Healthcare System (VAAAHS), which is a complex 1b medical center. The physical therapist (PT) is responsible for independently providing assessment and treatment interventions to inpatients and outpatients received through healthcare provider referrals and patient self-referrals. Responsibilities CARDIOPULMONARY AREA Serves as a consultant for all health care providers regarding Cardiopulmonary physical therapy and rehabilitation. Educates/orients staff regarding physical therapy in general and specifically as it relates to cardiopulmonary physical therapy. Mentors physical therapy staff and orients physical therapy students in cardiopulmonary physical therapy. Guides the work of the multidisciplinary cardiopulmonary team. Coordinates the inpatient, outpatient and HBCR programs regarding goal planning and discharge planning needs. Insures that all new cases are assigned to a physical therapy team member. Attends multidisciplinary care rounds and program planning meetings. In conjunction with the chiefs of Cardiology and Cardiothoracic Surgery, maintains and updates standard orders. Frequently reviews and updates cardiopulmonary physical therapy protocols and programs; manages the ordering and issuing of patient/program equipment for the HBCR program. Provides a minimum of one (1) in-service education program per year to the physical therapy department. Conducts competency training/testing for staff performing cardiopulmonary physical therapy evaluation and treatment. Plans, implements and reports on performance improvement activities relating to the cardiopulmonary physical therapy programs. Maintains, updates cardiopulmonary physical therapy monitoring data. Provides consultation and teaching for cardiopulmonary rehabilitation research projects. CLINICAL According to department policies, incumbent is authorized to perform complex examination and treatment of patients representing a wide scope of ages and diagnoses, rendering varied modalities inherent in their profession. Patient Evaluation- Demonstrates the advanced knowledge and skills necessary to provide treatment appropriate to patients despite complexity served on his/her assigned unit. Demonstrates knowledge of the changes associated with the aging/disease processes as well as possesses the ability to assess the patient's status to interpret the appropriate information needed to identify each patient's requirements relative to his/ser age-related, physical, psychosocial and cultural needs. Patient Treatment- After initial evaluation of patient, formulates progressive treatment plans and goals, and is responsible for implementation, modification, and documentation of the treatments. Apply advanced treatment methodologies and treatment theories in the provision of care to a vast array of functional impairment and patients, as well as provide clinical practice guidance for other staff physical therapists. Monitor and evaluate clinically appropriate treatment programs. Interdisciplinary Relationships- Communicates with, consults and works cooperatively with the VAMC disciplines, to provide comprehensive patient care. Requires advanced knowledge of contemporary physical therapy across a varied area of practice with the ability to apply this advanced knowledge to provide guidance to staff physical therapists within the department. Patient-Family Counseling/Education- Consults with patients and their families through patient/family education, training and advising them in the mature of their disability or disease in home programs and post hospital adjustment. Makes recommendations for home structural adaption when indicated. Documentation- documentation includes completion of consultation, initial notes, testing results, progress notes, discharge notes and treatment plans, patient education in accordance with policies (P.T., VA Joint Commission) for content and timeliness. Schedules patients, completes encounters, MDS, and closes consults in a timely manner. Age Specific- Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patient served (young adult, middle adulthood, Geriatric adult). Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education and experience. The individual must meet at least one of the requirements below: (1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. (2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist. (3) Doctorate degree in physical therapy. NOTE: Prior to 1995 the terminal degree for PT's was either a bachelors or master's degree. In 1995 CAPTE accredited all baccalaureate, master's, and Doctor of Physical Therapy terminal degree programs. Effective 2002, CAPTE changed its scope of accrediting activities of PT education programs to include only those programs that culminate in post-baccalaureate degrees(eliminating the baccalaureate degree). Effective 2015, professional physical therapy education programs have only offered the Doctor of Physical Therapy (DPT) degree to all new students. NOTE: Individuals must be a graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited college or university. The CAPTE is the only accreditation agency recognized by the United States Department of Education (USDE) and the Council for Higher Education Accreditation (CHEA) to accredit entry-level physical therapy programs. Verification of accredited programs may be obtained from the American Physical Therapy Association (APTA) at www.apta.org. Licensure. Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Non-licensed PTs, who otherwise meet the basic requirements in this standard, may be given a temporary appointment as a graduate PT at the GS-11 grade level under the authority of 38 U.S.C. 7405 (a)(1)(D) for a period not-to-exceed two years from the date of employment on the condition that such PT provide care only under the supervision of a PT who is licensed. Failure to obtain licensure during that period is justification for termination of the temporary appointment. NOTE. Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetency or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the state for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed physical therapists on VA rolls as of November 30, 1999, provided the individual maintains a continuous appointment and is not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. Loss of Credential. A PT who fails to maintain the required license must be removed from the occupation, which may result in termination of employment. At the discretion of the appointing official, an employee may be reassigned to another occupation, if qualified, and if a placement opportunity exists. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS- 13 Physical Therapist (Clinical Specialist) (a) Experience. In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level and one of the following: 1. Evidence of a minimum of 2,000 hours of clinical practice directly in the specialty area. This time must be between the past two and five years; Or 2. Board Certification in the specialty area recognized by the ABPTS; Or 3. Completion of a fellowship program; Or 4. An additional advanced degree in a related field. In addition, the candidate must demonstrate the following KSAs. (b) Demonstrated Knowledge, Skills, and Abilities 1. Advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of physical therapy. 2. Ability to serve as a consultant and subject matter expert for health care providers regarding the delivery of care within a specialized content area of physical therapy (see paragraph 2i). 3. Ability to teach and mentor medical center staff in a specialized content area of physical therapy. 4. Ability to guide the work of a multi-disciplinary team in a specialized content area of physical therapy. 5. Advanced knowledge and expert skill to perform and interpret specialized procedures and tests in evaluating the treatment outcomes in the assigned specialty content area (see Definitions, paragraph 2. i.). (c) Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety. Clinical Specialists spend at least 25% or greater of their time working with specialized population(s) within their department. Physical therapists at this level perform assignments in specialty areas such as geriatrics, neurology, cardiopulmonary, orthopedics, spinal cord injury, amputee, wound care, chronic pain, electromyography, women's health or pelvic health, polytrauma, brain injury, lymphedema, vestibular, assistive technology, seating and mobility. The individual is assigned responsibility for serving as the subject matter expert in the content specialty area and as a consultant to physical therapy and other medical center staff in evaluating and treating patients in the specialty area. Clinical Specialists have advanced knowledge and demonstrate expert clinical practice skills in providing assessment and treatment interventions across the continuum of care. He/she utilizes advanced knowledge and expertise to modify treatment plans and to identify therapeutic activity interventions based on the changing needs, goals and performance of patients. Clinical Specialists have advanced training in specialized technology related to their area of practice. He/she establishes and maintains contact with other health care providers involved in patient care via written, telephone, and personal communication on a regular basis. He/she exercises expert professional judgment to establish appropriate discharge planning recommendations to ensure safe discharge from inpatient or outpatient rehabilitation programs. If the medical center has a PT residency program, the clinical specialist may be on faculty for the program. Preferred Experience: Experience and skills in an acute care setting; experience and skills conducting Cardio Pulmonary assessment and intervention. Physical Requirements: This position frequently requires moderate to heavy lifting. Normally such lifting will involve, but not be limited to assisting with transfers or gait training of the client in the physical therapy clinic. Part of this position will involve sedentary work involving computers and communications.
