Summary Join our team! The Senior Social Worker within the Primary Care/Mental Health Integration Teams (PCMHI) operate within the Primary Care Clinic setting. The Senior Social Worker independently provides brief clinical psychosocial and case management services at an advanced practice level to eligible Veterans and their family members to meet biological, psychological, social, and environmental needs. Responsibilities Duties may include but are not limited to: The Senior Social Worker performs specialized treatment keeping in mind the impact of complex physical or mental illness. The Social Worker incorporates multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice The Senior Social Worker provides individual, group, and family psychotherapy and advanced level case management interventions used in the treatment of Veterans with polytrauma injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, addictions, and other serious disorders. Demonstrates the ability to teach and mentor staff and students in the special area of practice and to provide supervision for licensure or specialty certifications. This position requires frequent, direct contacts with patients to provide therapy and support. Frequency of contacts is increased in times of crisis. The Social Worker demonstrates flexibility of Veteran contact, providing face to face and virtual or telephonic appointments. The Senior Social Worker participates in team staffing by participating in the consult triage process, providing subject matter expertise on treatment recommendations. The Senior Social Worker displays skill regarding educating patients, diagnosis, symptomatology and treatment options. This position has the skill ability to provide time-limited Standard Episodes of Care, termination and aftercare. This position is trained in several Evidence Based Practice and flexibly utilizes them in their treatments. The Senior Social Worker takes a leadership position in communicating with their Interdisciplinary team, as well as across teams, working in close collaboration with clinic or program directors. They take the initiative to provide education regarding their role and the role of their program in Veteran care. In pursuit of programmatic and treatment goals, maintain effective working relationships and other professional and administrative personnel both within DVAHCS and in the community The Senior Social Worker participates in the training of Social Workers, Peer Support Specialists, volunteers, and students; participates as required and requested in orientation, training and staff development programs involving Social Workers across the Health Care System. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: GS12: Experience/Education: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: The Primary Care Mental Health Integration (PCMHI), Senior Social Worker must possess the ability to independently provide a full range of mental health services to Veterans and their family members enrolled in the Primary Care setting. Services include advanced mental health expertise and support to the Primary Care Clinic providers relative to Veteran patients experiencing primarily depression and/or problem drinking behaviors, although triage and initial psychosocial evaluation may occur related to other concerns such as thoughts of self-harm/suicide, anxiety disorders, adjustment disorders and grief/bereavement. Provision of brief evidence-based Standard Episodes of Care is inherent in this position as is functioning as a member of the PCMHI Team and the Primary Care Clinics. The Senior Social Worker must be able to take a leadership position in communicating with their interdisciplinary team, collaborate with clinic or program directors and participate in training of other social workers, trainees and/or medical staff.
