    Hours Per Week: 40 hours

     

    Hiring range: $77,646.40- $100,942.40 The actual compensation for this position will be determined based on experience and other factors permitted by law.

    Responsibilities:

    + Responsible for performing concurrent and retrospective clinical reviews to assure that evidence based best practices are implemented timely and documented appropriately, to include contemporaneous monitoring of severe sepsis and septic shock, Rapid Response activations and inpatient/outpatient cardiac arrest events.

    + Design, develop, and implement educational programs and training sessions, related to timely identification and management of clinical deterioration in hospitalized patients.

    + Responsible for the coordination and delivery of education and training on resuscitation to both clinicians and non-clinicians throughout St. Mary’s for American Heart Association (AHA) BLS, ACLS, and PALS courses.

    + Responsible for the abstraction of the CMS SEP-1 measure and data collection/ analysis of all Rapid Response and Code Blue activations.

    + Identifies opportunities to maximize the accuracy and effectiveness of clinical documentation for emergency response activations( RRT and Code Blue) as well as documentation requirements for the CMS- SEP-1 measure.

    + Facilitate the multidisciplinary Sepsis and Resuscitation Committees; schedule meetings, prepare agendas, compile reports, ensure representation from all roles in resuscitation performance.

    + Acts as a resource and provides ongoing consultation and education to internal customers and patient care teams, with respect to resuscitation performance improvement initiatives.

    + Uses clinical practice experience and knowledge of clinical workflow to help facilitate interdisciplinary teams engaged in patient safety improvement work related to resuscitation response and performance. Ensures national and state best practice standards are incorporated into project work plans.

    + Periodically reports on the organizations resuscitation performance and prepare reports for departments and committees.

    + Review/ revise organization policies related to resuscitation response and management ( Sepsis protocol, Rapid Response, Code Blue response).

    Additional Responsibilities:

    + Work closely with Nursing Education to provide mock codes and other education/ training for first responders.

    + Participate on various committees and work groups as assigned

    + Maintains schedules to comply with strict reporting deadlines

    + Other duties as assigned.

    Education, Training & Experience:

    + Bachelor’s degree in nursing required.

    + Experience with electronic/hybrid medical records.

    + Meditech experience preferred.

    + Previous experience with CMS Quality Measures reporting and Performance Improvement preferred.

    + Minimum of three (3) years’ experience specifically related to Quality Improvement activities.

    Certifications / Licensure:

    + Current New York State license as a Registered Nurse required

    + Certified Professional in Healthcare Quality (CPHQ) preferred
