We are hiring a skilled and compassionate physical therapist assistant to join our home health interdisciplinary team! You'll work in a home setting with a diverse patient population throughout these surrounding communities. Working outside of a medical facility, you'll have an opportunity to get to know patients as you guide their care in the comfort of their homes. Monday-Friday daylight shifts with rotations for on-call weekends.

Responsibilities:
+ Communicates patient’s status to the referring physician and other health care team members.
+ May serves as a clinical instructor for physical therapy students as well as residents and fellows in a specialty area of physical therapy.
+ Maintains professional license/registration through steady pursuit and participation in appropriate continuing education and professional development opportunities to satisfy requirements for licensure/registation renewal.
+ Performs physical therapy assessment, reassessment and treatment and supervises physical therapist assistants in the treatment of pediatric, adolescent, adult, and geriatric patients referred by a physician to the clinic.
+ Develops plan of care including goals and treatment plan under physician referral, nature of injury/illness, and results of physical therapy assessment.
+ Modifies or adjusts plan of care based on patient’s response to treatment and/or changes in patient’s status.
+ Works with PTs to adjust patient programs as patient status changes.
+ Rotates through selected treatment areas when necessary to facilitate organizational needs.
+ Attends and actively participates in staff meetings and in-service education programs.
+ Maintains written documentation for all physical therapy services rendered, including documentation of initial assessment, follow-up visits, and discharge summary consistent with department standards. Co-signs physical therapist assistant documentation biweekly.
+ Completes necessary charge and statistical information to accurately reflect the service rendered to the patient.
+ Provides verbal and/or written home instructions to the patient and/or family members when applicable.
+ Schedules patients to achieve and maintain a level of productivity which meets or exceeds the standard established for the facility by the organization while ensuring delivery of quality patient care.
Education: Must meet one of the following: A.1) Graduated from a physical therapist assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association; or if educated outside the United States or trained in the United States military, graduated from an education program determined to be substantially equivalent to physical therapist assistant entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or identified at 8 CFR 212.15(e); and2) Passed a national examination for physical therapist assistants. B. On or before December 31, 2009, meets one of the following: 1) Is licensed, or otherwise regulated in the State in which practicing.2) In States where licensure or other regulations do not apply, graduated on or before December 31, 2009, from a 2-year college-level program approved by the American Physical Therapy Association and, effective January 1, 2010, meets the requirements of paragraph (a) of this definition. Before January 1, 2008, where licensure or other regulation does not apply, graduated from a 2-year college-level program approved by the American Physical Therapy Association. On or before December 31, 1977, was licensed or qualified as a physical therapist assistant and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service. Experience: Two years of appropriate experience as a PTA are required. At least 2000 hours of patient-related acts and services that have been verified by a supervising licensed PT or employment as a PTA for at least three of the last five years immediately preceding the date of employment. Computerization Skills: Competency in computer skills in a Windows-based environment is preferred. Travel: Access to transportation in the Agency service area with a valid Pennsylvania driver's license and auto insurance
Licensure, Certifications, and Clearances:
CPR: RequiredCertification/Licensure: Licensed, registered or certified as a Physical Therapist Assistant by the state in which practicing.CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
