    Position: Full-Time Licensed Practical Nurse (LPN)Location: UPMC Hamot, 4 North Transitional Support UnitSchedule: 50/50 Nights/Days, Weekends/Holidays, 12-Hour Shifts

     

    UPMC Hamot, a Magnet-designated hospital, is seeking a Full-Time Licensed Practical Nurse (LPN) to join our 4 North Transitional Support Unit, an 11-bed unit that specializes in care for medically and surgically stable patients awaiting discharge. This unit offers a unique opportunity for nurses passionate about long-term care nursing to gain valuable experience in an inpatient hospital setting.

    Sign-On Bonus:

    + $12,500 for nurses with over 1 year of experience.

    + $10,000 for new graduate nurses.

     

    _Title and salary will be determined based upon education and nursing experience._

     

    The Licensed Practical Nurse (LPN) provides care and IV therapy following UPMC guidelines and State Board regulations, participates in planning, implementing, and evaluating nursing care under the direction of an RN or other licensed UPMC provider, and collaborates with the healthcare team to achieve patient outcomes. Senior LPNs must complete an annual contribution to improve patient care or maintain a UPMC-approved certification and take nurse assignments at the top of their license in med/surg/tele or Emergency Department settings.

    Responsibilities:

    + LPN licensed to function as a health-care team member using sound nursing judgment.

    + Participates in planning, implementation, and evaluation of nursing care using focused assessment.

    + Communicates with licensed professional nurse and health care team when: Patient's care needs exceed LPN scope of practice. Patient's care needs surpass LPN's knowledge, skill, or ability. Patient's condition deteriorates, changes significantly, or needs immediate assistance.

    + Obtains instruction and supervision for new or unfamiliar practices/procedures. Follows UPMC policies, UPMC LPN Practice Guidelines, and State Practice Act.

    + Acts within LPN Practice guidelines, including IVs, and Relationship Based Care framework. Formulates daily goals and care plans involving the patient holistically.

    + Communicates patient's condition to other care providers. Comprehensive patient documentation promotes caregiver communication.

    + Incorporates national, business unit, and health system goals to improve patient safety, quality, and satisfaction.

    + Creates compassionate, patient-focused experiences by building healing relationships.

    + Actively participates in unit activities and assists care team members. Provides detailed teaching to patients and families for care transitions.

    + Supports orientation and development of new LPNs and patient care support staff. Seeks feedback and applies continuous professional improvement.

    + Participates in organizational and system programs for continuous learning. Identifies opportunities for clinical quality and workplace improvement.

    + Participates in policy and procedure development for patient comfort and safety. Seeks and participates in process improvement opportunities.

    + Develops and maintains productive working relationships, demonstrating accountability, enthusiasm, and commitment. Understands cultural differences and holds peers accountable for healthy relationships.

    + Maintains balance between work and personal life, models safe work hours, time management, and healthy lifestyle.

    + Communicates workplace safety hazards to peers and management.

    + Performs in accordance with system-wide competencies/behaviors. Performs other duties as assigned.

    + Successful completion and graduation from State Board-approved Licensed Practical Nursing Program. 2 years of experience required.

    + IV therapy curriculum completion required within 4 months of hire/transfer.

    + Commitment to obtain IV therapy curriculum within 4 months of hire/transfer within 30 days of start.

    + Graduates from 2012-present: IV therapy curriculum included in PA State Board Approved Programs. Graduates prior to 2012: Provide official proof of IV therapy approved program completion.

    + Nurse assignment at top of LPN license in med/surg/tele or Emergency Department setting.

    + Select and complete 2 contributions annually to improve patient care OR maintain UPMC approved certification and complete 1 contribution.

    + Demonstrate knowledge and skills for age-specific patient care. Understand principles of growth and development over the lifespan.

    + Assess data reflective of patient's status and interpret information for age-specific needs. Identify process improvement opportunities for patient care.

    + Collaborate with health care team, patients, and families for desired outcomes.

    + Participate in policy and procedure development for patient comfort and safety.

    + Establish and maintain positive relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.

    + Work productively and effectively in complex environments, handle multiple/changing priorities and specialized equipment. Mobility and visual manual dexterity required.

    + Physical stamina for frequent walking, standing, lifting, and positioning of patients. Computer skills required for electronic health record utilization.

    Licensure, Certifications, and Clearances:

    + Advanced Cardiac Life Support (ACLS) within one year of hire/transfer.

    + Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

    + Licensed Practical Nurse (LPN) must maintain state licensure where they are practicing or in a state covered by a licensure compact agreement with the state where the facility is located. If graduated prior to 2012, the LPN will need to provide official proof of completion of IV therapy approved program within 4 months of hire/transfer.

    + NIH Stroke Scale (NIH) within 6 months of hire/transfer.

    + Act 34

    *Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
