Brighton Ave, Syracuse, NY 13205.Our Syracuse location is comprised of 2 buildings; the Cunningham building, a 13-floor, 487-bed long-term care skilled nursing facility, and Fahey Rehab, a 96-bed short-term residential rehabilitation program - the largest in Central New York. Our Fahey building offers specialized services including Palliative Care and Restorative Care.     TheCharge Nurse, RN, is responsible for performing a variety of duties to provide quality nursing care and to coordinate total nursing care for residents. Under the guidance and direction of the Nurse Manager, participates in planning, organizing and reviewing operations, implements procedures and programs; determines work procedures and expedites work flow; ensures compliance with all operating policies, procedures, and regulations. Communicates with and supervises appropriate personnel; provides daily and other reports, and performs direct care duties using accepted nursing best practice standards when required

    Responsibilities

    Resident/Client Care Nurse – Summary

     

    + Provides direct nursing care and assists with procedures, treatments, and examinations.

    + Monitors resident health, administers medications, and ensures accurate implementation of physician orders.

    + Supports residents with ADLs and special care needs, while promoting independence and respecting individual rights.

    + Communicates with families and caregivers, responds to emergencies, and collaborates with interdisciplinary teams on care planning and hospital transfers.

     

    Documentation

     

    + Maintains accurate, timely care plans and medication records. Collects and documents resident health data and communicates updates to the care team in accordance with regulatory standards.

     

    Compliance

     

    + Follows all nursing policies, procedures, and third-party regulations. Completes required trainings, health assessments, and competencies.

     

    Employee Development

     

    + Supports new staff orientation and in-service education. Provides guidance and sets care priorities for team members.

     

    Equipment & Property

     

    + Ensures medication carts and medical equipment are properly stocked and maintained. Reports maintenance issues promptly.

     

    Quality Assurance

     

    + Monitors quality of care, supports performance improvement activities, and addresses noncompliance with care plans as needed.

     

    Corporate Participation

     

    + Actively contributes as part of the interdisciplinary care team.

    Qualifications

    + Graduate of an accredited school of nursing. BSN or BS in related field preferred.

    + Current NYS license and registration as an RN in good standing required.

    + Proven patient assessment and care planning skills.

    + Current knowledge of best practice nursing standards and care.

    + Ability and knowledge to respond in emergency situations.

    + Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.

    + Ability to lead, guide and motivate staff.

    + Outstanding interpersonal and communication skills, including the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.

    + Ability to manage multiple priorities in a busy environment.

    + Proficient in MS Office programs and able to learn new software rapidly.

     

    Benefits/Compensation

     

    + Pay Range $38.51 - $50.75 / hour including shift differential
