    Job Summary

    The Permanency Specialist II Family delivers permanency services within Hillside’s Permanency Resource Centers and/or Kinship programs. The Permanency Specialist II will work primarily with adult caregivers including Kinship caregivers, legal guardians, and adoptive parents. Services are delivered through in-person visits, educational and support groups, and family enrichment activities. These services seek to increase connections and support for families in order to sustain permanency for the youth in their care.

     

    This is a full-time (40 hours) position primarily on a Monday-Friday 8:30a-5:00p schedule, but flexibility for some occasional evening & weekend hours is required.

     

    Essential Job functions

     

    + Provide case management services for families adhering to funder deadlines and compliance requirements.

    + Engage caregivers to increase supports, connections, and skills in order to positively impact permanency for youth in their care.

    + Complete all client enrollment processes within specified time frames to include: initial intake, assessments, treatment plans, and safety plans.

    + Schedule and meet with assigned families, in person, for one-to-one monthly client visits.

    + Assess family needs and assist families in identifying and accessing needed resources in order to support permanency on a continuous basis.

    + Provide parenting education one-to-one and in group format to help caregivers manage challenging, high risk youth behaviors at home, in the community and school.

    + Attend school meetings when needed to advocate for youth needs and support parent/caregiver.

    + Work with team to identify and implement quarterly family engagement activities and educational workshops.

    + Develop, maintain, and update all required documentation according to agency policies and protocols.

    + While this job description covers many aspects of the role, employees may be required to perform other duties as assigned.

     

    Education & Experience

     

    + Bachelor’s degree in social work, psychology, or human services; OR Bachelor’s degree in any field with a minimum of 2 years of case management experience required

    OR

    + Associate’s degree in psychology or human services with a minimum of 2 years of case management experience required.

    SPECIAL REQUIREMENTS

    + Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.

     

    Knowledge, Skills & Abilities

    In addition to demonstrating the Hillside Professional Competencies of Communicates Effectively, Personal Excellence, Cultural Competence, Builds and Leverages Relationships, and Optimizes Decision Making, the following occupational competencies must be demonstrated:

    + Ability to understand and adhere to all funder requirements

    + Ability to deliver parenting education training

    + Ability to continuously assess client needs and adapt plans accordingly to address identified needs

    + Ability to de-escalate and manage crisis situations both in-person and by phone

    + Ability to work through a series of issues using independent judgment

     

    Physical Demands & Work Environment

    + Must be able to work a variable scheduling including evenings and weekends

    + The following daily physical demands are required:

    + Sitting (up to 6 hours)

    + Occasional standing (up to 2 hour)

    + Occasional walking (up to 2.5 hours)

    + Driving (several times a week up to 6 hours)

    + Continuous balancing (up to 8 hours)

    + Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)

    + Weekly lifting up to 10-15 lbs. on a daily basis

    + Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)

    + Occasional exposure to dust, fumes, gases, chemicals, or smoke is apparent

    + Ability to change positions as needed

    SPECIAL CONSIDERATIONS

    While Hillside is a restraint free environment, there may be times in a “life or limb” situation, that staff may be required to physically restrain clients weighing between 50 lbs. and 300 lbs. and guide them safely to the floor. In some circumstances, restraints can last up to 15 minutes and may require repetition as necessary.

     

    $21.40 Minimum pay rate, $31.00 Maximum pay rate, based on experience.
