Summary:
The Medication History Pharmacy Technician is responsible for obtaining an accurate medication history for patients admitted to the hospital and enters medication history obtained into the EMR.
Responsibilities:
The Medication History Pharmacy Technician assists pharmacists, nurses, and physicians with medication reconciliation to ensure an accurate medication history.

Qualifications
EDUCATION REQUIREMENTS
High School Graduate. Two-year associate degree preferred.
PROFESSIONAL EXPERIENCE REQUIREMENT
One to two years of pharmacy retail experience or equivalent.
LICENSURE/REGISTRATION/CERTIFICATION
Certified Pharmacy Technician (CPhT) or obtained within one year of hire.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIREMENTS
Basic knowledge of medications.
Strong verbal communication skills.
Good written communication skills.
Basic math skills.
Computer data entry skills.
Good organizational skills and ability to prioritize responsibilities.
Must be able to talk with patients/caregivers and others to obtain and provide information in a professional manner.
Ability to work independently and to make decisions based on patient requirements.
WORK ENVIRONMENT
Potential exposure to blood/body fluids or chemicals requiring the use of personal protective equipment.
Potential stressful working conditions and rotating shifts.
Clean, well-lit comfortable climate with moderate noise level.
PHYSICAL REQUIREMENT
Pushes and pulls carts, standing, walking, bending, stooping, and reaching with arms throughout shift.
Close visual ability, good depth perception and physical hand and finger dexterity, listening and communication skills.
Requires lifting of weights of 20 pounds.
