    Job Summary and Qualifications

     

    The PPS Coordinator promotes high quality, patient-centered care by providing clinical support in the prompt, accurate completion of of the IRF Patient Assessment Instrument (IRF-PAI), to ensure accuracy in the scoring of functional status and reflection of the patient’s care needs. In collaboration with the care team, the PPS Coordinator also assists in the ongoing monitoring of identified benchmarks for the inpatient rehab population. The PPS Coordinator accesses confidential patient information and maintains confidentiality of all information.

    What you will do in this role:

    + You will oversee and ensure the accuracy and completion of the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI).

    + You will maintain current knowledge of IRF-PAI software.

    + You will collaborate with physicians and interdisciplinary team members to capture accurate assessment of patient.

    + You will participate in ongoing education to the staff to ensure prompt, accurate completion of the IRF-PAI.

    + You will participate in internal chart reviews and external audit coordination.

    What qualifications you will need:

    + Associate Degree in Nursing or Respiratory Therapy OR

    + Bachelor’s Degree in Physical or Occupational Therapy OR

    + Master’s Degree in Speech Therapy required

    + Currently licensed as a registered professional nurse or other licensed professional in specialty and/or has an active compact license.

    + Basic Life Support (BLS) required

    + 2+ years experience in an acute hospital setting required

    + 2+ years experience in inpatient rehabilitation preferred

    + Certified Registered Rehab Nurse (CRRN) preferred

    + Advanced training in specialty preferred
