Responsibilities

Lead the delivery of quality patient care and adherence to policies, procedures and case management protocols.

+ Actively participate in the process of monitoring, guiding and evaluating nursing staff performance.

+ Responsible for coordinating activities, prioritizing workflow, and promoting decision-making and problem solving.

+ Serve as a first point of escalation for team members needing consultation for and/or management of patients requiring complex or advanced treatment/care.

+ Provide onboarding support to new associates. Functions as a preceptor to new associates and works with management to develop and adjust the orientation plan.

+ Provide input into hiring, training, directing, development and evaluation of staff as well as day-to-day operational support to management as needed.

Requirements

Licensure / Certification / Registration:

Required Credential(s):

+ Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

Education:

+ High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degree required.

Work Experience:

+ 1 year of experience required.

Additional Preferences

Licensure / Certification / Registration:

Preferred Credential(s):

+ Case Manager credentialed from the American Case Management Association (ACMA).

+ Case Manager credentialed from the Commission for Case Manager Certification (CCMC)

Education:

+ Master's degree preferred.
