    Description

    Jet Health Inc. / Trio Hospiceis seeking an experienced and dynamicCommunity Outreach & Education Navigatorto serve as a liaison between the Agency and the community, focusing on education, outreach, and engagement in ourDenton, TXhospice office. This role is responsible for promoting awareness and understanding of hospice and home health services through public education, relationship building, and community partnerships, while supporting the mission and values of the Agency.

    Requirements

    + Develop and deliver community education sessions on hospice and home health care topics.

    + Establish and nurture relationships with senior centers, faith-based organizations, community leaders, and local agencies.

    + Plan, coordinate, and attend outreach events, health fairs, and educational forums.

    + Collaborate with Agency teams to ensure consistent messaging and branding in community engagement efforts.

    + Act as a community resource and advocate for patient choice and access to care.

    + Identify and support opportunities to improve public awareness and understanding of end-of-life care and chronic condition support.

    + Provide input on community needs and trends to inform program development.

    + Assist in building partnerships and collaborations with other local healthcare providers and organizations.

    + Ensure compliance with all local, state, and federal laws related to outreach and education.

    + Attend community/networking/social events and represent the organization professionally.

    Qualifications

    + Graduate of an accredited college/university is preferred.

    + Background in healthcare, social work, public health, or education is beneficial.

    + Must possess excellent communication, presentation, and interpersonal skills.
